The Breast

한글 유  ·  Pinyin Ru

What a man keeps in the kidney a woman keeps here — milk that will not come and milk that will not stop, the blown breast, the abscess, and the lump that hardens over ten years

乳

The chapter opens with a symmetry: a man's root is the kidney and a woman's the breast, above and below differing but the root of life the same. Then the practical half. Milk that will not flow is of two kinds — too full to pass, or too little to come — and the first is opened while the second is fed, with trotter broth, tetrapanax and a wooden comb drawn down the breast thirty strokes at a time. Milk that comes before the birth is called weeping breast; milk in a woman with no child to take it is dried with barley sprout. The middle sections are mastitis: the breast belongs to the yang brightness and the nipple to the reverting yin, so anger blocks the duct and rich food heats the blood until it suppurates, and the chapter is emphatic that a knife must not be used. The last section is the hardest. A lump that grows through years of grief and swallowed anger, painless and unmoving, which after ten years or more sinks into an ulcer with a hollow like a cave — 妳巖, breast rock. The Bogam says it can be treated only while it is still a lump, that once it has broken open nothing will answer, and that a woman past fifty with an abscess in the breast is better left alone to finish her natural years.

Where a woman has borne many children and has no milk, it is because the fluids are lost, and enriching and boosting medicines should be taken to stir it. Where there is milk but not much of it, medicines that free the channels should be taken to stir it, drawn on with broths and stews. For a woman's milk draws on the penetrating vessel, which communicates with the stomach channel. In general, where a woman has a long-standing disease in the penetrating and controlling vessels, the milk is scant and yellow in colour and the child she bears is timid, weak and often ill.

Translation영역
The breast is where the yang brightness passes, and the nipple belongs to the reverting yin. Where a nursing mother does not know how to regulate and nourish herself — anger counterflowing, depression and oppression barring the way, rich flavours feeding it — the reverting yin's blood does not move, so the orifice shuts and the milk does not pass; and the yang brightness's blood boils, so the heat is severe and turns to pus. There are also cases where the suckling child has stagnant phlegm in the diaphragm and hot breath, and falls asleep with the nipple in its mouth, so that the hot qi blows on it and a hard core forms — this is called blown breast. At the very beginning one must bear the pain and knead it until it is somewhat soft, and have it sucked so that the milk comes through, and it may disperse of itself. Miss this and leave it untreated, and it will surely become an abscess.
Clinical note임상 주석
忍痛按令稍軟, 捻令汁透 — bear the pain, press until it softens, and squeeze until the milk comes through. That is the whole treatment of an early blocked duct, and it is what is taught now. The name 吹乳, blown breast, comes from the notion that the sleeping child's hot breath blows into the nipple.

Where the infant cannot draw the breast, or the child's breath has blown on it, or at weaning the milk was not all squeezed out, in every case the milk stands and gathers within and strikes against the blood and qi: at first there is swelling and pain, then a hard knot, and when it comes to the hand not being able to come near, it is called jealous milk.

Translation영역
After the birth the milk should be diligently squeezed out and not allowed to gather. Gathering and not being drawn off, it binds into foul milk within, drawing heat and fever, binding hard with a pulling pain, with great thirst and drinking, the breast swollen with urgent pain and the hand unable to come near — this makes jealous milk, and it is not an abscess. Quickly moxa Eoje on both hands fourteen cones, which cuts off the abscess pulse. Then the hand no longer hurts to come near the breast, the milk comes out of itself, and one may help it out by squeezing, so that the milk pours out, all of it like pus. Inwardly take Forsythia Decoction that is, Five Fragrances Forsythia Decoction, formula in the Welling Abscess chapter, and outwardly smear on powdered adzuki bean, and it mends.
Clinical note임상 주석

Translation영역
It mostly comes of the phlegm of rich flavours and damp heat standing in the diaphragm and striking against stagnant milk. There are also cases made by the child's breath blowing on it, and others engendered by anger rousing and stagnating. Calcined gypsum, burnt birch bark, trichosanthes seed, licorice nodes and green tangerine peel are all divinely effective medicines. In a woman, if this disease is treated early it disperses at once. While the menses are still coming it is all light disease; after fifty or sixty when there are no menses, it must not be looked on as easy.
Clinical note임상 주석

2.14 Breast Disease Differs in Man and Woman 男女乳疾不同

Translation영역
Breast disease in a man differs slightly from a woman's: in a woman the liver and stomach are damaged, in a man the liver and kidney. Anger-fire and bedchamber exertion past measure bring the liver to dryness and the kidney to deficiency, and this too can bind into a lump, or make it swell and pain. Sixteen Flavours Flow the Qi Drink and Clear the Liver and Resolve Depression Decoction are appropriate both formulas above.
Clinical note임상 주석
The same lump, reached by a different route. A woman's breast disease is read through the liver and stomach because the stomach channel runs through the breast and milk is made of grain qi; a man has no such traffic there, so what is left is the liver channel and the kidney behind it, and the cause named is anger and sexual exhaustion rather than stagnant milk. The treatment is accordingly the same two formulas the chapter has already given for bound breast lumps, not a separate class of medicine.

Gypsum (石膏): Brings down the milk. Take 2 nyang, decoct it in water and take, three times a day (Boncho 本草). ○ At the first onset of a breast abscess: calcine it, powder it fine, and take 3 don stirred into warm wine. (Jikji 直指)
Clinical note임상 주석
The section answers two complaints and almost nothing else: milk that will not come, and a breast that has become hot and hard. 乳癰, breast abscess, is mastitis and its abscess, and the chapter's word for its early stage is 妬乳, the jealous breast — an engorged, blocked, inflamed breast in a nursing woman. The modern management is to keep the breast emptied, by feeding or expressing, with antibiotics for an established mastitis and drainage for a collected abscess; an abscess will not resolve on a poultice.

Chinese yam (山藥): The fresh root treats the blown breast with swelling and pain. Pound it to a mash and apply it, and the swelling disperses; take it off quickly, for fear the flesh should rot.
Clinical note임상 주석
速去之, 恐肉腐 — take it off quickly, for fear the flesh should rot. A stated time limit on a poultice, and a reason given, which is rare. Fresh yam mucilage is irritant enough to blister thin skin, and the compiler had evidently seen it happen. 吹乳, the blown breast, is the chapter's third term for the same condition, and the image is of air blown into the breast and swelling it.

Clinical note임상 주석
欲成癰, about to become an abscess — the chapter is careful about the stage, and the distinction matters as much now as then: an inflamed breast that has not yet collected can settle with emptying and antibiotics, while one that has needs draining.

Clinical note임상 주석
**Rhinoceros horn is not to be used.** All rhinoceros species are on CITES Appendix I, the trade is prohibited, and demand for the horn is what has driven them to the edge of extinction; buffalo horn is the substitute in licensed preparations. 酒磨, ground in wine, is the preparation reserved for the hardest animal materials, which will not give anything up to boiling.

Clinical note임상 주석
罷隨手欲睡, 是其功也 — afterwards one wants to sleep at once, and that is the drug working. A stated expected response, and a humane one: the patient is told that the drowsiness is part of it and not a worry. Clinical note임상 주석
腫硬如石, swollen and hard as stone, is the breast of a severe engorgement or an established abscess, and 不通, not flowing, is the key to it: the milk is not getting out. Green tangerine peel is the corpus's drug for a breast blocked by constraint, and it is still the one reached for where the trouble is read as stagnation rather than heat.

寒熱, chills and fever, is the systemic part of the picture, and it is the sign that turns a blocked breast into an infection needing antibiotics. 熱卽換, change it as soon as it grows hot, is a cooling compress used properly: it is changed when it stops being cool.

Clinical note임상 주석
令人嗍却黃水 — have someone suck out the yellow fluid, which is the one instruction in the section that addresses the actual problem. A breast that is not being emptied is what keeps a mastitis going, and emptying it, by feeding or expressing, is still the first thing to do. The antler paste is incidental.

Pig's trotters (猪四蹄): Move the milk vessels in a woman (Boncho 本草). ○ Where a woman who has given birth is depleted in qi and blood and has no milk at all: four pig's trotters and 4 nyang of rice-paper plant, boiled together in 1 mal of water down to 4 or 5 seung; take the liquid and drink it continuously. After drinking, take a wooden hair comb and comb downward over the breast — effective. (Dansim 丹心)
Clinical note임상 주석
The best entry in the section, and the one still in use: pig's trotters boiled with rice-paper plant is the classic Korean and Chinese prescription for a poor milk supply, and it is a rich gelatinous broth given to a woman who is depleted after childbirth. The comb stroked downward over the breast is massage toward the nipple, which is exactly what is taught now for a blocked duct — the comb is a tool for doing it evenly.

Wild boar fat (野猪脂): Treats a breast with no milk. Take a spoonful, mix it with a jan of warm wine and take it, three times a day, and the milk comes. Moreover the milk is plentiful, enough to supply five children.
Clinical note임상 주석
可供五兒, enough for five children, is the section's most cheerful claim. Alcohol in fact reduces the milk ejection reflex and passes into the milk, so wine three times a day is the wrong vehicle for a nursing woman; the fat itself is simply calories, which a woman feeding a baby does need.

Cat's fur (猫兒毛): Treats the taxation breast abscess, ulcerated so that the core is visible. Take the fur from the belly, burn it with its nature preserved and powder it, add a little calomel, and apply it mixed with clear oil.
Clinical note임상 주석
**Calomel is not to be used.** It is mercurous chloride and it is absorbed through broken skin; applied to an open ulcer it goes straight into the circulation, and this is the most dangerous prescription in the chapter. 乳勞癰爛見心者 — a breast abscess of long standing, ulcerated down to the core — is a chronic breast abscess, a tuberculous breast, or an ulcerating carcinoma, and a breast ulcer that does not heal needs a biopsy. Twenty simples, and the ones to keep are the two that treat the mechanics: emptying the breast, and the trotter broth for a mother who is not eating enough.
