癕, 為, 温 and 𥂁 are given above as 癰, 爲, 溫 and 鹽, and 醫鍳 as 醫鑑. The transcription leaves a space before 各等分 after 甘草. The alternative name 千金內托散 recorded here is not Naetakcheongeum-san 內托千金散, nor Cheongeumnaeso-san 千金內消散, nor the 千金托裏散 listed in the method leaf; the four names are distinct prescriptions in this chapter. 木香湯 is the aucklandia decoction used as the vehicle and is not an ingredient.

Translation

Treats every kind of abscess, deep-rooted sore, sore and boil: where it has formed it opens quickly, where it has not formed it scatters quickly, spoiled pus comes out of itself, malign flesh falls away of itself, and it stops pain, expels pus and grows flesh; its effect is like a spirit's. Ginseng; astragalus (steeped in salt water, then steamed and dried); Korean angelica (wine-washed); magnolia bark (ginger-processed); platycodon; cinnamon bark; cnidium; saposhnikovia; angelica dahurica; licorice — in equal parts. Powder them, and take 3 don at a time stirred into warm wine. For those who do not drink wine, send it down stirred into an aucklandia decoction. — Jeongyo 精要. Also called Cheongeumnaetak-san, the Thousand-Gold Inner Expression Powder. — Uigam 醫鑑. Also called Baenongnaebo-san, the Expel Pus and Supplement Within Powder, and also called Hobyeokdowi, Commandant Who Guards the Wall; taking it removes the old and grows the new. — Deukhyo 得效. It is suited to the cold of winter; in the summer months Naetakbokjeon-san is the better choice. — Immun 入門 (Donguibogam · Ongjeo)

Clinical commentary 臨床

The template of the chapter's supporting arm, and the formula the method leaf puts first when it lists what to use for 托裏: Sipseon-san, Gamisipgi-san, Cheongeumnaeso-san, Takrisodok-san, and the rest. Ten drugs in equal parts, which is what the name counts. Ginseng and astragalus supplement the qi, Korean angelica and cnidium the blood, cinnamon bark warms, and saposhnikovia, angelica dahurica, platycodon and magnolia bark move outward and open — so the prescription supplements and pushes at the same time, which is precisely what 內托 means. It is the exact counterpart of the chapter's draining group, Naesohwangryeon-tang, Oridaehwang-tang, Cheongeumnuro-tang and Hwadok-dan, which assume a strong patient and carry the toxin out through the gut. Here the patient is assumed spent, and the swelling is to be thrust up and out instead. Two of the chapter's other leaves are defined against this one: Naetak-san is this plus white peony, Gamisipgi-san is this plus frankincense and myrrh. The alternative names are worth keeping, and one of them is a trap. 千金內托散 is this formula; 內托千金散 and 千金內消散 are two others in the same chapter, and the method leaf lists a 千金托裏散 as well. 排膿內補散 describes what it claims to do. 護壁都尉, Commandant Who Guards the Wall, is a military title, and Gamisipgi-san's other name, 固壘元帥, Marshal Who Secures the Rampart, is its deliberate twin — the body's surface as a fortification to be held. That is pedigree and metaphor, not pharmacology, and it belongs in the history. The 入門 note at the end is the practical one: suited to winter cold, with Naetakbokjeon-san preferred in summer, which is a seasonal substitution between two formulas of the same group. The modern reading of all of them is the same. A wound with slough and thin pus in a run-down patient does need the patient built up, and nutrition genuinely bears on wound healing; it does not get better because warm wine and a powder were taken three don at a time, and dead tissue is removed surgically. — Eui Young (Justin) Chung, L.Ac., Dipl.
