Translation

Treats an abscess or deep-rooted sore whether it has formed or not; taking it disperses the swelling inwardly, and it is especially suited to those of waning years and weak qi. Korean angelica, cinnamon bark, ginseng, astragalus, cnidium, angelica dahurica, saposhnikovia, platycodon, magnolia bark, licorice, frankincense and myrrh, in equal parts. Powder the above, and take 3 don at a time stirred into warm wine. For those who do not drink wine, send it down stirred into an ophiopogon decoction. Also called Goryu Wonsu, Marshal Who Secures the Rampart. It is the Sipseon-san above with the two drugs frankincense and myrrh added. — Deukhyo 得效 (Donguibogam · Ongjeo)

Clinical commentary 臨床

Sipseon-san plus frankincense and myrrh — the resin pair the tradition reaches for when a swelling is painful — and the indication shifts accordingly. Where Sipseon-san is written for the sore and Naetak-san for the hollowed-out patient after it opens, this is written for the old and the weak, and for the swelling that is to be dispersed inwardly before it ever comes to a head. The twelve drugs are listed in a different order from the parent leaf but they are the same ten with the two additions. It sits with the rest of the 托裏 and 內托 group on the supporting side of the chapter's central pair. Ginseng and astragalus build the qi, Korean angelica and cnidium the blood, cinnamon bark warms, and the surface-opening drugs push outward; nothing here drains downward, and that is what separates the whole group from Naesohwangryeon-tang, Oridaehwang-tang and Cheongeumnuro-tang, which precipitate the toxin out of a repleted body and take free movement of the bowel as their measure. Which arm a patient belongs to is decided by how much strength there is to work with. The alternative name is the twin of Sipseon-san's. 固壘元帥, Marshal Who Secures the Rampart, answers 護壁都尉, Commandant Who Guards the Wall — the body's surface imagined as a fortification and the formula as the officer holding it. That is a sixteenth-century way of talking about resistance to infection, and it is history rather than mechanism. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
