內托千金散

癕 is given above as 癰. 桂皮 is registered under 肉桂, with the part named in the prep, and 瓜蔞根 has its own entry. This formula is not Sipseon-san, whose alternative name is 千金內托散, nor Cheongeumnaeso-san 千金內消散, nor the 千金托裏散 listed among the 托裏 prescriptions in the method leaf; the four are distinct.

Translation

Treats every kind of abscess, deep-rooted sore and malign sore; it is able to express from within. Honeysuckle, ginseng, astragalus, red peony, Korean angelica, cnidium, trichosanthes root, angelica dahurica, cinnamon bark, platycodon, saposhnikovia and licorice, 1 don each. Cut the above, make one packet, decoct in water and remove the dregs, stir in half a cup of wine and take, three doses a day. Afterwards black blood issues from the mouth of the sore, or sweat comes out over the whole body; this is the drug having taken effect. — Dansim 丹心 (Donguibogam · Ongjeo)

Clinical commentary 臨床

Twelve drugs flat at 1 don each, which gives the formula no hierarchy at all and makes it read as a composite rather than a built prescription. Ginseng, astragalus, Korean angelica, cnidium and cinnamon bark are the 內托 core carried over from Sipseon-san; honeysuckle and trichosanthes root come from the heat-clearing side; angelica dahurica, platycodon and saposhnikovia open outward. So the leaf sits with the supporting arm of the chapter's central pair, opposite Naesohwangryeon-tang, Oridaehwang-tang, Cheongeumnuro-tang and Hwadok-dan, which assume a repleted patient and carry the toxin down through the bowel. Here nothing drains, and the whole design is to push a swelling up and out of a body that has been given enough to push with. The names in this cluster are a genuine hazard and worth setting out once. 千金內托散 is Sipseon-san's other name. 內托千金散 is this leaf. 千金內消散 is a third formula, closer to Seonbanghwalmyeong-eum. And the method leaf's list of 托裏 prescriptions names a 千金托裏散 as well. Four names built from the same four characters, and four different prescriptions. The closing sentence is the interesting one. Black blood from the mouth of the sore, or sweat over the whole body, is counted as proof the drug has worked — the toxin leaving by the two routes the chapter trusts, the sore itself and the skin. Read clinically, dark blood from a sore is old blood or necrotic tissue and tells you about the wound rather than about the remedy, and an abscess still needs to be drained. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
