聲音

A short chapter with an unusually exact anatomy. The voice has three organs behind it: the heart masters it, the lung is its gate, the kidney its root — so a voice lost suddenly is treated at the lung and one lost slowly at the kidney. The Lingshu then names the parts that make speech and gives each a job: the epiglottis is the door of the voice, the lips its leaves, the tongue its trigger, the uvula its gate-pin, and the hyoid the bone the spirit qi works the tongue with — and a small thin epiglottis opens and shuts easily while a large thick one stammers. The second section is diagnosis by ear, from the Nanjing's five notes to the Immun's five phases of sound, and Dongwon's observation that a voice from a damp body sounds as if spoken from inside a jar. Then the two kinds of muteness that the whole chapter turns on: tongue muteness, where the tongue will not move but the voice is unchanged, and throat muteness, where the voice is hoarse but the tongue works — the first a stroke, the second consumption. The formulas divide the same way, and three medicinals run through nearly all of them: chebula to hold the voice, platycodon to open it, licorice to ease it.

The heart is the master of the voice, the lung its gate, the kidney its root. Where wind, cold, summerheat or damp, or qi, blood, phlegm or heat, have offended against the heart and lung, the disease is in the upper cavity; resolve it according to the pattern, and when the evil qi disperses the heaven-pipes sound again. But where the kidney's vacuity is the disease, it cannot draw the various qi in and return them to the origin, and so the qi counterflows upward: there is cough with phlegm congestion, panting or distension, and chest, abdomen and the hundred bones are all pulled by it. The more the cough worsens, the more the qi fails, and the drier the voice becomes.

Three organs and a rule for dividing the chapter: an acute voice is the lung's and a chronic one the kidney's. 天籟, the pipes of heaven, is the Zhuangzi's phrase for the wind in the hollows, borrowed for a throat that has come open again.

A child had vomiting and diarrhoea, and Qian Yi gave Trichosanthes Decoction formula in the Children chapter and it was healed. Then the child passed neither stool nor urine; another physician used medicine to free them, and the child turned cold-bodied and would not eat or drink. Qian Yi used Boost the Yellow Powder and Quisqualis Pill, and sure enough the body warmed and the child could eat and drink. Then the child would not speak, and Qian Yi said: this comes of cooling medicines used to free the urine, so that spleen and kidney are both made vacuous. Now the spleen is already replete and the kidney still vacuous — he used Six Flavours Rehmannia Pill formula in the Vacuity Taxation chapter, and in a month it was healed.
Clinical note임상 주석
Qian Yi's case, in three moves, and the last is the chapter's argument in practice: a child who has stopped speaking after over-purging is treated at the kidney, because 3.1.1 puts the root of the voice there. The warning about cooling medicines in children is the Children chapter's whole quarrel with the physicians of his day.

Three voices and three locations, and the third is the best of them: a thin drawn-out voice for a disease in the head, which is what a person with a headache sounds like when they are trying not to jar it.

Ten organs with a quality of voice each — the Nanjing's 聞而知之 worked out to the bowels as well as the viscera. 肝病則聲悲, the liver's voice grieving, sits oddly against the Liver chapter's 2.4, where the liver's sound was shouting.

The earth sound is as though one spoke from inside a deep jar. It is also said: where damp prevails, the voice is as though it came out of a jar — the damp being like water in the centre.

如從瓮中出 — as if from inside a jar. The muffled, resonant voice of a blocked nose and congested sinuses, and Dongwon reads it as damp because damp is what dulls a sound. It is the chapter's most usable piece of listening.

Shaoshi answered: the gullet is the road of water and grain. The windpipe is what the qi goes up and down by. The epiglottis is the door of the voice. The mouth and lips are the leaves of the door. The tongue is the trigger of the voice. The uvula is the gate-pin of the voice. The nasopharynx is where the divided qi is discharged. The hyoid is what the spirit qi works with, and governs the moving of the tongue. So where a person's nose runs and the mucus cannot be held back, the nasopharynx is not open and the divided qi is lost. Therefore where the epiglottis is small and thin the qi is issued quickly, its opening and shutting is easy, and the breath comes out readily; where the epiglottis is large and thick the opening and shutting is difficult and the breath comes out slowly, and so there is stammering. When a person suddenly has no voice, cold qi has lodged in the epiglottis, so that the epiglottis cannot issue, and issuing cannot come down; its opening and shutting does not meet, and so there is no voice.

The Lingshu's 憂恚無言 chapter, and the anatomy of speech is nearly right: epiglottis, lips, tongue, uvula, nasopharynx and hyoid, each with a function, and the hyoid correctly given as what the tongue is worked from. 重言, doubled speech, is stammering, and the Lingshu puts it down to a thick epiglottis.

瘂風, mute wind — a stroke that has taken only the speech, with everything else intact. The Wind chapter treats it; the acorus is added here because acorus is what the tradition used to open the heart's orifice, and 3.1.1 made the heart the voice's master.

Where the reversal qi runs to the throat and one cannot speak, with the hands and feet cold and the stool not free, take the foot lesser yin. It is also asked: when a person suddenly has no voice, how is it needled? Qi Bo said: the foot lesser yin ties above to the tongue, networks to the hyoid and ends at the epiglottis. Drain its blood vessels on both sides and the turbid qi is driven off. The epiglottis's vessel networks above to the controlling vessel; take it at Cheondol a point name, and the epiglottis then issues.

The kidney channel traced to the tongue, hyoid and epiglottis — the anatomical basis for 3.1's claim that the kidney is the voice's root — and Cheondol (天突, CV22), in the suprasternal notch, is where the needle goes.

Three cones of moxa were burnt at each of the two Pungnyung points and one at each of the two Johae points, and his voice came out at once. Then scutellaria was used to downbear the fire as sovereign, apricot kernel, tangerine peel and platycodon to drain the reversal qi as minister, chebula to discharge the counterflowing qi and licorice to harmonize the origin qi as assistants — and taking it he was well healed.

Moxa first and the formula after, and the formula is written out by rank — sovereign, minister, assistants — which is the chapter's one worked example of how a prescription is built. Pungnyung (豐隆, ST40) for phlegm and Johae (照海, KI6) for the throat are still the standard pair.

Muteness is evil entering the yin division. The Neijing says: where evil strikes against the yin there is muteness. It also says: where evil enters the yin and strikes, there is muteness. But there are two patterns. The first is tongue muteness — the sort where in wind-stroke the tongue will not turn and move.

The second is throat muteness — the sort where taxation cough has taken the voice. In tongue muteness the root of the tongue cannot turn and move for speech, but the voice in throat and gullet is as before; in throat muteness there is only hoarseness in the throat, while the root of the tongue can still turn and move for speech.
Clinical note임상 주석
The chapter's central distinction, and it is a clean one: dysarthria against dysphonia. Tongue muteness is a stroke and is treated at the Wind chapter; throat muteness is laryngeal and is what the rest of this chapter treats. A physician who could tell them apart by asking the patient to speak had most of what mattered.

Where the five viscera cough long there is hoarseness. Hoarseness is the throat broken; it is not a disease of the gullet's gate.

喉破也, 非咽門病也 — the throat broken, not the gullet diseased. The Deukhyo separating the larynx from the pharynx, and it is the same distinction 3.6 drew: what hurts to swallow and what has taken the voice are different places.

After illness with vomiting and diarrhoea, or after a great illness, where there is voice but the patient cannot speak, and yet can still take medicine — this is not loss of voice, but kidney timidity unable to reach up and join the yang. The kidney should be supplemented, and Rehmannia Pill formula in the Vacuity Taxation chapter governs it. Loss of voice is a sudden illness from contracting wind cold, and that is all.

雖有聲而不能言 — there is voice but no speech. Qian Yi separating a third thing from both kinds of muteness: the exhausted child who can make a sound and has not the strength to form words. It is neither a stroke nor a laryngitis, and treating it as either would be wrong.

Where one cannot lie down and the breath has a sound, this is the yang brightness counterflowing. The three foot yang channels run downward; now they counterflow and run upward, and so the breath has a sound. The yang brightness is the stomach's channel; where the yang brightness counterflows it cannot follow its road, and so one cannot lie down. Where one's rising and resting are as usual and yet the breath has a sound, this is the lung's network vessels counterflowing; a disease of the network vessels affects a person slightly, and so rising and resting are as usual and yet the breath has a sound.

Two noisy breaths told apart by one question: can the patient lie flat? Orthopnoea with noisy breathing is the stomach channel — the serious case — and noisy breathing in someone otherwise well is the lung's networks, which is minor. The Suwen's triage is sound.

Throat ulceration with aphonia in a wasting patient is tuberculous laryngitis, and the Worms chapter's 14.20.3 gave the same verdict: 勞瘵咽瘡失音者死. The two chapters agree, and they are describing the end of the same disease.

Treating a man who suffered a dry cough with a hoarse voice: white atractylodes 2 don; ginseng and red tangerine peel 1 don 5 pun each; pinellia leaven 1 don; white poria, mulberry root bark and asparagus 7 pun each; licorice and green tangerine peel 3 pun each; schisandra 20 grains; anemarrhena, lycium root bark, trichosanthes seed and platycodon 5 pun each — made one packet with 3 slices of ginger and decocted. In summer add 5 pun of scutellaria. Along with it, Four Substances Decoction with child's urine, bamboo sap and ginger juice, and fried phellodendron; the two medicines taken alternately by day and night. In two months the voice came out and he was healed.

Danxi's case, and the method is two formulas alternating day and night — one to supplement the qi and dry the phlegm, one to nourish the blood and cool the fire — for two months. The chapter ends its treatments on a patient who took that long.

Twelve simples for the voice — acorus, akebia fruit, tetrapanax, apricot kernel, cinnamon heartwood, bitter bamboo leaf, tangerine peel, pear, dried persimmon, sesame oil, child's urine and hen's eggs. Most are taken by holding them in the mouth and swallowing the juice, as the chapter's formulas are. The pear entry is for wind-stroke muteness: press the juice and take a hop twice a day.
