脇

The flank is where the liver and gallbladder channels spread, and so a pain there is the liver's until shown otherwise. The chapter's method is to name the causes and treat each: qi depressed, dead blood, phlegm rheum, food accumulated, wind cold — and then two more that cut across them, vacuity against repletion and left against right. The pulse tells the first: a stringlike pulse on both hands is liver qi in surplus. The history tells the rest. A pain that presses worse when pressed and comes at night is old blood; one that comes with cough and a pulled breath is rheum; one that rises like a ridge under the ribs after a heavy meal and hard work is food. The left-and-right rule is the chapter's most-used: Bitter Orange and Chuanxiong Powder for the left, Push the Qi Powder for the right, three medicinals each, differing in one. The chapter closes away from pain altogether — on fox-smell under the arm, treated with snail and croton or simply with ginger juice rubbed in daily for a month, and on leaking armpits, where the palms and soles are always damp enough to spoil the clothes.

The three names mark off the side of the chest from the shoulder to the last rib, and the chapter uses them precisely: 腋 for the hollow, 脇 for the ribbed side, 季脇 for the floating ribs where the liver is felt.

謀慮不決 — plans and deliberations that will not be settled. The Liver chapter made the liver the general who decides, and the Gallbladder chapter the official of decision; here the failure to decide is given as a cause of pain in their own territory.

按之則痛益甚 — worse for pressing. The one sign that separates a blood pain from a qi pain, which eases under the hand. It costs nothing to test and the chapter relies on it.

乾脇痛 — a dry flank pain: one fixed point that never lets up, in a wasted patient. The Immun calls it 甚危, very grave, and treats it by supplementing. A constant unremitting point of pain in someone losing weight is a presentation any clinician takes seriously.

A man suffered flank pain and the physicians took it for a welling-abscess. The yang pulse was stringlike and the yin pulse rough; the aromatics, ginger and cinnamon they gave made it worse. Xiang Xin saw him and said: stringlike means pain, rough means the kidney's evil is in surplus. The kidney presses upward against the flank and cannot go down; and the kidney hates dryness, yet he has taken too many drying medicines — nothing but a purge will heal it. First Spirit Protecting Pill was used and black urine came down and the pain stopped; then Divine Chuanxiong Pill was taken. Some doubted this was too much, and Xin said: Spirit Protecting Pill was used because the kidney's evil had pierced the membrane, and nothing but scorpion can lead a medicine there; but croton is hot by nature, and without mirabilite and rhubarb to wash it out, it will break out again on meeting heat. So there was a great purging several times, and the disease healed.

A case argued rather than reported: why the first physicians were wrong, why the purge was necessary, and why a second purge was needed after the first. Xiang Xin's reason for following a hot drastic with a cold one — that the heat left behind will restart the disease — is the kind of second-order reasoning the case records rarely give.

Also called armpit qi, and also fox stink. At the fifth watch take two large slices of lean pork, coat them with a nyang of powdered kansui, and hold them in the armpits until daybreak; then drink a decoction of a nyang of licorice. After a good while foul matter is purged out — this must be done out in open country, for fear the foul qi passes to others. Done so three to five times it heals. As for litharge, ceruse and the like, they only stop up the orifice and treat the branch.

The kansui is absorbed from the armpit and purges; licorice, its classical antagonist, is drunk to meet it. 皆塞竅以治其末 — the powders only stop the opening and treat the branch: the Hoechun is distinguishing a deodorant from a cure, and choosing the harder route. **Kansui is toxic. The pork is coated and held against the skin, not eaten.**

Keep a large pond snail alive in water; when its operculum opens, lift a single croton kernel on a needle and put it inside the snail, and set it face up in a cup. In summer one night, in winter five to seven, and it turns of itself to a liquid; take this and smear it under the armpit, and the root is cut off.

耳內有油濕 — oily damp inside the ear. Wet earwax and axillary odour are governed by the same gene, and a person with one very often has the other. The tradition noticed the association and used it as a diagnostic sign.

Another method: first smear honeyed fat under the arm, and the place from which the fox stink issues turns yellow; then take the croton and snail as above, remove the operculum, cover the spot with it and bind it tight with silk. The fox stink comes out in the stool, and the root is cut off.

The one method here a person could use: ginger juice rubbed in daily for a month. Ginger is antibacterial, and axillary odour is made by bacteria acting on sweat — so of all the chapter's remedies this is the likeliest to have done something.

Seven simples — green tangerine peel, immature bitter orange, bitter orange, inula, saposhnikovia, cattail pollen and fresh ginger. The green tangerine peel entry carries the chapter's one caution about its own favourite: 靑皮乃肝膽二經之藥, it is the medicine of the liver and gallbladder channels and right for an angry person with depression under the ribs — but where the qi and blood of those two channels are insufficient, supplement the blood first and use only a little of it. The last entry is for fox stink, not for pain: ginger juice rubbed under the arm.
