Registered here under 控涎丹, the head name at 6.10.1 where the composition is given. The Fright Phlegm section at 6.5.9.1 lists the same formula as 妙應丹 and refers the composition forward with 方見下; both passages point to this entry. Momordica seed, scorpion, realgar, pangolin scale and the other medicinals named in the modifications are not entered as ingredients, since they belong to variant forms rather than the base formula. Pangolin scale is a CITES-listed species and prohibited in trade.

Donguibogam 東醫寶鑑

一名妙應丹, 治痰飮流注作痛. 甘遂ㆍ紫大戟ㆍ白芥子 各等分. 右爲末, 糊丸梧子大, 曬乾, 臨臥, 薑湯, 或溫水下七丸至十丸, 神效. 驚痰, 加朱砂爲衣. 痛甚, 加全蝎. 酒痰, 加雄黃. 臂痛, 加木鱉子ㆍ桂心. 驚痰成塊, 加穿山甲ㆍ鱉甲ㆍ玄胡索ㆍ蓬朮. 《河間》

Translation

Also called Myoeung-dan. Treats pain from phlegm-rheum flowing and lodging. Kansui, purple euphorbia and white mustard seed in equal parts. Powder them, make paste pills the size of a firmiana seed and sun-dry. At bedtime take seven to ten down with ginger decoction or warm water, with marked effect. For fright phlegm, coat them with cinnabar. For severe pain, add scorpion. For alcohol phlegm, add realgar. For arm pain, add momordica seed and cinnamon heart. For fright phlegm bound into a mass, add pangolin scale, turtle shell, corydalis and curcuma. — Hagan 河間 (Donguibogam · Damum)

Clinical commentary 臨床

Three medicinals, two of them drastic. The Deukhyo passage that introduces it at 6.10 is one of the most striking in the chapter: patients with unbearable shifting pain in the chest, back, limbs and hips whom ordinary physicians misread as wind stroke and treat with wind medicinals and needling to no effect, or mistake for an incipient abscess and plaster uselessly. The text says flatly that all of this is wrong, that the cause is phlegm-drool lying above and below the diaphragm, and that this formula alone will lift it. The associated signs are worth holding — a sawing sound in the throat at night, copious drool, cold numb hands and feet, heavy sleepiness, no taste for food. **Kansui and euphorbia are drastic purgatives and are toxic. This formula is not suitable for self-administration, is contraindicated in pregnancy and in depleted patients, and the classical incompatibility with licorice applies.** The dose given is seven to ten pills, which for a formula of this class is already at the limit. — Eui Young (Justin) Chung, L.Ac., Dipl.
