Kiln Earth Salve

Yoto-go

한글 요토고  ·  Pinyin Yao Tu Gao

窰土膏

久年窰竈土 and 竈心黃土 are the kiln-and-stove earth and the yellow earth from the heart of the stove; both are 伏龍肝 and the drug is entered under bongnyonggan, whose alias list carries 竈中黃土. The transcription prints 竈, which the corpus also prints, and a stray space before 各五錢, read as nothing. The oiled paper and the binding cloth are the dressing and are kept in the method. 茶淸, the clear tea of the preliminary wash, is a plain liquid and is likewise kept in the method. The appended ○ is a 丹心 commentary on the composition rather than a further prescription.

Translation

Treats the outer shin ulcer. Kiln and stove earth of many years, or else the yellow earth from the heart of the stove, with phellodendron, red halloysite and red lead, 5 don each; calomel, frankincense and myrrh 1 don each. Powder the above and work it to a salve with sesame oil. First wash with clear tea, then spread the medicine on oiled paper, apply it, and bind it fast with a cloth. Even if the itching is unbearable, wait until a scab has formed before taking it off; if it is not healed, apply it again. — Immun 入門. ○ Stove-heart earth dries the damp and clears the heat, the pungency of phellodendron disperses the fire evil, and frankincense and myrrh disperse the stagnant Blood: it may be called complete in every respect. — Dansim 丹心 (Donguibogam · Jechang)

Clinical commentary 臨床

Old kiln and stove earth at five don, with phellodendron, red halloysite and red lead at the same weight and a don each of calomel, frankincense and myrrh, worked to a paste with sesame oil, spread on oiled paper and bound over the ulcer. The appended 丹心 note explains the design drug by drug, which is unusual and worth having: the stove earth dries and cools, phellodendron disperses the fire, the resins move the stagnant Blood. Fired earth and red halloysite are both, in effect, absorbent clays, and an absorbent clay dressing on a weeping ulcer is a real and old idea. The instruction to leave it alone through the itching until a scab forms, 縱痒不可忍, 直待結痂去之, is good dressing discipline — not disturbing a healing ulcer is most of the battle — and the itching it anticipates is the surrounding venous eczema that goes with these ulcers. Against that, the materials. **Earth from a kiln or stove floor applied to an open ulcer is a tetanus and clostridial risk and introduces soil organisms into a chronic wound; it is the one ingredient here that could kill. The salve also contains red lead and calomel, lead and mercury salts that are absorbed through ulcerated skin and accumulate over the months such an ulcer takes. This must not be prepared.** The chapter's own advice at the head of the section is the part to keep: 須翹足端坐, 勿多行履 — sit with the leg up and do not walk about. Elevation and compression heal venous ulcers; the dressing only keeps them clean. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
