Yellow Beeswax Salve for the Shin Ulcer

Hwangnap-go

한글 황랍고  ·  Pinyin Huang La Gao

黃蠟膏

The id takes the -jechang tail because the bare id hwangrap-go is already held by the 黃蠟膏 of the 得效 line in the 皮門, a four-drug wax salve for winter chapping of the hands and feet; the two share a name and nothing else. 油髮 is entered under 頭髮 by way of the 油髮 alias, and 如梅大, the size of a plum, is a size measure and takes no gram figure. 香油 is entered under 淸油 by way of the 香油 alias. The silk binding and the porcelain storage jar are apparatus and are kept in the method.

Translation

Treats the shin ulcer, inner and outer alike. Sesame oil 1 nyang; put in oiled hair the size of a plum and cook it until it dissolves; put in sweetgum resin 3 don and beeswax 1 nyang and let them melt; put in dragon bone, red halloysite and powdered dragon's blood resin 3 don each, stir until even, and when it has cooled store it in a porcelain jar. Each time, pinch off a thin sheet, lay it on the sore, bind it fast with silk, and after three days turn it over and apply it again. — Immun 入門 (Donguibogam · Jechang)

Clinical commentary 臨床

A wax salve stiffened with minerals. Sesame oil and beeswax at a nyang each are the base; sweetgum resin adds tack; dragon bone, red halloysite and dragon's blood resin at three don each give it body and astringency, red halloysite being an absorbent clay and dragon's blood a haemostatic resin. The lump of oiled hair dissolved in the hot oil is the one odd item and contributes charred keratin. The handling is the good part: the salve sets hard enough in its jar that a thin sheet can be pinched off and laid on the ulcer like a patch, bound with silk, and turned over after three days. That is a wax dressing with a three-day change interval, used over both the medial and the lateral ulcer, and it is a clean occlusive — moist, covered, undisturbed, which is what a chronic leg ulcer needs. The chapter also uses it as the finishing dressing after 爐灰膏 has been used to eat away dead flesh. There is no heavy metal in this leaf, which makes it one of the two plasters in the section worth reading seriously. **The caution is contamination rather than toxicity: burnt hair and a re-used jar of salve applied repeatedly to an open ulcer invite infection, and the whole dressing should be sterile.** And the dressing is not the treatment — 須翹足端坐, 勿多行履, sit with the leg raised and do not walk about, is the chapter's own advice at the head of the section and it is still exactly right for a venous ulcer. — Eui Young (Justin) Chung, L.Ac., Dipl.
