蠟礬紙

槐枝, pagoda-tree twig, has no entry in the herb list and is left out of the ingredient list and named here; it is cooked in the oil and the dregs discarded, so it contributes only what the oil takes up. 枯礬 is calcined alum and is entered under 白礬, whose alias list carries 枯白礬. 麻油 is entered under 淸油 by way of the 麻油 alias. The transcription prints a stray space before 一錢, read as nothing. The cotton paper, the paper twist and the binding cloth are the dressing itself and are kept in the method. 四十九粒 and 四十九寸 are count and length measures and take no gram figures. The 藥水 of the preliminary wash is unspecified in the leaf.

Translation

Treats the inner shin ulcer, with divine effect. Stack twelve layers of cotton paper, look at the size of the sore and cut them into a square patch, and pin them together with a twist of paper. Separately take sesame oil 2 nyang, put in forty-nine grains of Sichuan pepper and cook it black over a gentle fire, discarding the dregs; put in forty-nine inches of pagoda-tree twig, boil it until withered and black, and discard the dregs; put in beeswax 1 nyang, calcined alum 1 don and calomel 2 fun, and wait until they melt. Then put the paper prepared earlier straight in and let the oil soak right through it, taking care not to let it scorch yellow, and lift it out. When applying, first wash and wipe the part with the medicinal liquor, lay the sheets of paper on together, and bind them tight with a cloth; after a full day take off the one sheet next to the sore. When the paper has all been taken off, the sore is wholly healed. — Immun 入門 (Donguibogam · Jechang)

Clinical commentary 臨床

A medicated dressing rather than a salve, and the most ingenious thing in the chapter. Twelve sheets of cotton paper are cut to the shape of the ulcer and pinned into a pad; sesame oil is cooked with Sichuan pepper and pagoda-tree twig, the dregs thrown away, and beeswax, calcined alum and calomel melted into it; the pad is soaked through with the hot medicated wax and lifted out before it scorches. It is bound on, and every day one sheet — the one against the sore — is peeled away. When the last sheet comes off, the ulcer is said to be healed. Peeling one layer a day is a clean, graded dressing change: the sore is never left open, the pad is never disturbed down to the base, and the paper count gives the course a fixed length of about a fortnight. A paraffin-gauze dressing works on the same principle and is still in use. Beeswax and oil keep the surface moist, alum astringes the weeping edge, and the twigs and pepper contribute whatever the oil has taken from them. 內膁瘡, the inner shin ulcer, is the medial gaiter-area ulcer, which is where venous ulcers overwhelmingly sit — the chapter has sorted its leg ulcers by the anatomy that actually matters. **The dressing contains calomel, a mercury salt, which is absorbed through ulcerated skin and accumulates over the weeks and months a leg ulcer takes; omit it, and the wax-and-alum paper is left, which is the useful part.** The chapter's own 須翹足端坐, 勿多行履 — sit with the leg raised and do not walk about — remains the treatment, with the dressing in support. — Eui Young (Justin) Chung, L.Ac., Dipl.
