Pinch of Gold

Illyeomgeum

한글 일념금  ·  Pinyin Yi Nian Jin

一捻金

The leaf carries no composition of its own — it is 賽命丹, registered as `saemyeong-dan`, ground to a powder instead of rolled into pills — so no ingredients are entered. The interlinear cross-reference 方見上 is set as a sochu span, the form this chapter uses. Donguibogam carries two other formulas whose names romanize alike, 一捻金散 of the Uihak Jeongjeon and 一捻金元 of the Seui Deukhyobang, registered as `illyeomgeum-san` and `illyeomgeum-won`; this bare 一捻金 of the Abscesses chapter is a third and needs no tail. The leaf stands in the transcription as though it were a subsection heading of 疔疽, but it is a formula name and is entered as one.

Donguibogam 東醫寶鑑

治疔疽. 卽<a href='/formulas/saemyeong-dan'>賽命丹</a><span class='sochu'>方見上</span>爲末. 每服二三分, 溫酒調下. 如服賽命丹後毒未盡起, 再用此藥催之. 服此藥後身凉者卽死. 《入門》

Translation

Treats clove sore. It is Saemyeong-dan 賽命丹 — the formula is given above — made into a powder. Two or three fun to a dose, stirred into warm wine. If after taking Saemyeong-dan the toxin has not fully risen, use this drug again to urge it on. If the body turns cold after this drug has been taken, the patient dies. — Immun 入門 (Donguibogam · Ongjeo)

Clinical commentary 臨床

A dosage form rather than a formula: the same 賽命丹 of the upper part of the chapter, powdered instead of pilled, and given at two or three fun — a pinch, which is what the name says. The point of the change is speed. Pills have to disintegrate; a powder stirred into warm wine is absorbed at once, and the leaf uses it precisely when 賽命丹 has already been given and the toxin has not come up, 催之, to urge it on. The last clause is the part worth keeping: 服此藥後身凉者卽死 — if the body turns cold after this is taken, the patient dies. Read as prognosis it is accurate. A patient with a spreading deep infection who goes cold and shut down is in septic shock, and that was as lethal in the Bogam's hands as the leaf says. Read as pharmacology it is also a warning the text does not quite make: this is a powdered dose of a pill built on toad venom, cinnabar, realgar, arsenic and chalcanthite, and going cold after swallowing it is at least as likely to be the drug as the disease. **Being 賽命丹 in powder form, this contains toad venom, cinnabar, realgar, arsenic, chalcanthite, centipede and scorpion, with musk and pangolin scale besides, and a powder is absorbed faster than a pill. It must not be prepared or taken.** It is recorded for textual completeness. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
