한글 연연진인탈명단  ·  Pinyin Yuan Ran Zhen Ren Duo Ming Dan

淵然眞人奪命丹

眞 and 寶 are written 真 and 寳 in the transcription, and 醫鑑 is written 醫鍳; all are given in the corpus forms. 隂, 𤍠 and 為 are given as 陰, 熱 and 爲. The interlinear notes (乾則酒化), (連殼用) and (酒灸) are given in parentheses, the corpus reserving 《》 for source labels. 葱白 is registered under 蔥白 and 菉豆 is the corpus form; the mung bean here is only the size of the pill and is not an ingredient, and the flour paste that binds them when the snail paste will not is kept in the source text only, as the corpus does elsewhere. 灸 for the roasting of the centipede is the form this corpus prints throughout. 麫 is given as 麪.

Translation

Treats specifically every kind of effusion on the back, the yin-type deep-rooted sore, the nail-sore, the malign sore and unnamed toxic swelling. Take it and the swelling rises and effuses; where there is a head but no pain, take it and the pain comes; where it has already formed, take it and it is cured at once — this is the chief treasure among the drugs for malign patterns. Realgar, 3 don; toad venom (dissolved in wine if it is dry), frankincense, myrrh and verdigris, 2 don each; dragon's blood resin, chalcanthite and calcite, 1 don each; calomel, musk and borneol, half a don each; snails, 21 of them (used with the shells); centipede, one, wine-roasted. Powder the above, grind the snails to a paste, and bind into pills the size of a mung bean; if they will not bind, boil flour paste in wine and bind the pills with that, and coat them with 2 don of powdered cinnabar. Only two pills to a dose. First take 3 inches of scallion white, have the patient chew it soft and spit it into the palm of the hand — the left for a man and the right for a woman — put the pill inside the scallion, and send it down with three or four cupfuls of hot wine. Cover the patient with clothing, and after about the time it takes a man to walk five li, give several more cups of hot wine to help the strength of the drug; sweating freely with heat effusing is the measure. If no sweat comes, give two more pills. — Dansim 丹心. Also called Biryongtalmyeong-dan, the Flying Dragon Snatch-Back-Life Elixir. — Uigam 醫鑑 (Donguibogam · Ongjeo)

Clinical commentary 臨床

Among the chapter's many formulas named for a person or a lineage — the Perfected Man Yeonyeon here, Dongwon, the Venerable Elder, the Great Unity, the Mica — this one wears its pedigree hardest, and the leaf's own line about it being the chief treasure among the drugs for malign patterns is advertisement rather than indication. The history belongs in the record; the claim does not. What is actually prescribed is an attacking pill, not a supplementing or a draining one, and it sits apart from both arms of the chapter's central pair. Realgar leads, with toad venom, frankincense, myrrh and verdigris behind it, then dragon's blood resin, chalcanthite and calcite, then calomel, musk and borneol in half-don amounts, and twenty-one snails ground to a paste doing the work of a binder. The administration is the most elaborate in the chapter: two pills only, wrapped in a three-inch length of scallion white the patient has chewed soft and spat into his own palm, sent down with hot wine, the patient covered up, and more hot wine after the time it takes to walk five li. Free sweating is the endpoint, which makes this a diaphoretic in intent — the toxin is to be driven out through the skin rather than up or down. The three indications are precise and worth reading: a swelling that will not rise, a headed sore that does not hurt, and a sore already formed. An abscess that does not hurt and will not come up is the dangerous presentation, then as now, and the leaf is right to single it out. **Contains realgar, which is an arsenic sulfide; cinnabar and calomel, which are mercury compounds; chalcanthite and verdigris, which are copper salts; toad venom, which contains cardiac-glycoside bufadienolides and has killed people at ordinary doses; and centipede. Musk is a CITES-protected material. Hot wine on top of all of it speeds absorption. This must not be prepared or taken, and no substitution makes it safe.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
