Marvellously Effective Snatch-Back-Life Elixir

Sinhyotalmyeong-dan

한글 신효탈명단  ·  Pinyin Shen Xiao Duo Ming Dan

神效奪命丹

褁 is left as the transcription prints it and 䓗 is given as 葱, the corpus form. The collation note 䓗 : 葱 at the page boundary of leaf 010a stands after two leaves that each write 䓗 — this one's 生䓗白 and the 蟾酥丸's 䓗酒 — and the first edition's reading 葱 applies to both; the note is recorded in the 蟾酥丸 entry as well. 一字 is a quarter of a 錢 and is entered as 1 g. The twenty snails are a count item and carry no weight. 梧子大, the firmiana seed that gives the pill its size, is method rather than an ingredient.

Translation

Treats clove sore and malign sore, and lockjaw from a wound where the patient is sunk in stupor and in imminent danger. Cinnabar 3 don 5 fun, used as the coating; calomel and dragon's blood resin, 2 don each; alum calcined to dryness and toad venom, 1 don each; verdigris a quarter don; snails, twenty. Powder the above and bind with the milk of a woman nursing her first son into pills the size of a firmiana seed, coated with the cinnabar. Have the patient chew several stalks of raw scallion white himself and spit them out, wrap one pill in them, and send it down with warm wine. If he retches, force it down with hot wine; the whole body sweats and the toxic Qi disperses of itself. — Dansim 丹心 (Donguibogam · Ongjeo)

Clinical commentary 臨床

A 奪命 pill of the usual shape, with one indication the others in the family do not carry: 破傷風, lockjaw after a wound, with the patient sunk in stupor. That is tetanus, and putting it in the same leaf as clove sore makes sense within the chapter's logic, since both are read as a wound toxin striking inward. The toad venom and the sweating method are the same as in 返魂丹 and 淵然眞人奪命丹; what differs is the binder. The binder is the leaf's distinguishing feature and its most uncomfortable one — 初男乳汁, the milk of a woman nursing her first son, which the Donguibogam specifies in this exact form elsewhere for 何首烏丸. The choice is sympathetic, the first milk of a first son being the purest and most yang; it carries no pharmacology. The administration is the family's, with one practical addition: if the patient retches the pill back, hot wine is used to force it down, which in a patient already stuporous is a clear aspiration risk. **Contains cinnabar and calomel, which are mercury compounds; verdigris, a copper salt; and toad venom, whose bufadienolides act on the sodium-potassium pump exactly as digoxin does and have killed people at ordinary doses. It is bound with human milk and forced down with hot wine in a patient who may be unconscious. This must not be prepared.** Tetanus is prevented by immunisation and treated with antitoxin, wound debridement, antibiotics and intensive supportive care; a spreading clove sore needs antibiotics and surgery. — Eui Young (Justin) Chung, L.Ac., Dipl.
