Quiet the Worms Powder (Children chapter)

Angchung-san

한글 안충산 (소아)  ·  Pinyin An Chong San

安蟲散

The leaf carries no source label of its own. The 腹痛腹脹 section prose names 安蟲散 and 安蟲丸 together and closes with 《錢乙》, so the entry is attributed to Jeoneul and the source label is left out of `source_trans`, which ends with the chapter tail alone. The name is also an unflagged homonym: 安蟲散 is already registered as `angchung-san`, the 局方 powder of the Worms chapter, and that collision is invisible both to the batch's dup list, which keys on the raw glyphs 安虫散 against 安蟲散, and to `hanrom.py`. The two are not the same prescription. The 局方 powder has no composition of its own — 右以化蟲丸材爲細末 — so it is 化蟲丸 reground, which gives 胡粉, 白礬, 檳榔 and 苦練根 at 5 don and 鶴虱 at 3 don, dosed at 1 don in rice water for anyone. This leaf names the same five drugs explicitly and puts them all at 2 don, dose one ja or 5 fun graded by the size of the child, with a pill form and an alternative name the 局方 entry does not have. Same five drugs, different proportions, different dose, different lineage — a homonym, so it takes the chapter tail. Glyph forms follow the corpus majority: 蟲 is printed for the transcription's 虫 (120 occurrences against 8) and 苦練根 is kept as the transcription and the corpus both write it (12 against 1).

Clinical commentary 臨床

Qian Yi's worm powder, and the one place in this section where the name of the formula and the disease actually meet. Three of the five drugs are genuine anthelmintics and two of them are still in use: **areca (檳榔) and chinaberry root bark (苦練根) are real vermifuges**, carpesium (鶴虱) is another, and what they treat is **ascariasis** — roundworm. The section's description of the child is a good clinical picture of it: crying and writhing with pain about the heart and belly, vomiting clear water and foam, the face blue-yellow, the pain coming and going, and the lips purple-black. A heavy worm load really does cause colic, and a bolus of worms can obstruct the bowel or block the bile duct. So this formula was aimed at the right target, and today **a single dose of albendazole or mebendazole does the same job** — safely, cheaply, in one swallow, with cure rates over ninety per cent. That is the honest comparison. **Contains ceruse (胡粉), which is white lead — lead carbonate — in a formula given to an infant. This must not be prepared.** Lead is a developmental neurotoxin with no safe level of exposure; in a small child it costs cognitive ability permanently, and the gut absorbs several times more of it than an adult's does. The alum adds nothing good. The dosing here is better than most of the chapter — one ja for a small child, 5 fun for a bigger one, five pills for a one-year-old — so the text does grade it by the child's age and size, but **it gives no dose by weight here**, and a lead compound has no dose at which it is safe to give a baby anyway. — Eui Young (Justin) Chung, L.Ac., Dipl.
