Construction and Defence Soul-Returning Decoction

Yeongwibanhon-tang

한글 영위반혼탕  ·  Pinyin Rong Wei Fan Hun Tang

榮衛返魂湯

癕 is given as 癰 and 䓗 as 葱, the corpus forms; 脉, 並 and 盖 are left as the transcription prints them. The collation note 䓗 : 葱 standing at the page boundary of leaf 006b annotates this leaf's 䓗白 in the modification clause, and is attributed in the transcription to the 重刊本, the middle edition, rather than to the 初刊本 as the other notes of this chapter are. The 1.12 癰疽通治藥 section cites this formula again with ≪方見上≫, a cross-reference to this leaf, so no second entry is made for it. 內補十宣 in the closing clause names two formulas, 內補散 and 十宣散, rather than one.

Translation

Governs every complaint arising from phlegm-rheum and treats phlegm swelling specifically; it also treats abscess and deep-rooted abscess, back effusion, and the toxic swellings of flowing-and-lodging. Red peony, akebia, angelica dahurica, fleeceflower root, bitter orange, fennel fruit, lindera, Korean angelica and licorice, 1 don each. Cut the above, make one packet, and decoct in half wine and half water and take. — Uirim 醫林. ○ It is also called Chupungtonggi-san, the Chase the Wind and Free the Qi Powder, also Tongsun-san, the Freeing and Smoothing Powder, and also Hasuo-san, the Fleeceflower Root Powder. This formula should be used in alternation with Sipseon-san, with honeysuckle vine added to both. — Immun 入門. ○ Whenever Qi and Blood run counter in the grain of the flesh and are made to congest and knot into an abscess, this drug is greatly able to make the Qi move smoothly and the Blood run even, so that construction and defence of themselves become free and smooth, no changed pattern arises, and it suits the inward abscess especially well. — Immun 入門. ○ For vacuity add blast-fried aconite; for repletion add rhubarb; when phlegm is abundant add arisaema and pinellia; when a toxic swelling is hard and will not break add cnidium, ephedra, scallion white, whole scorpion and pangolin scale; for flowing-and-lodging add pubescent angelica. In flowing-and-lodging the Qi and Blood are congealed and stagnant, and adding pubescent angelica can set the Blood vessels of the whole body in motion; once the Blood vessels are moving, how should there be any flowing-and-lodging left? — Immun 入門. ○ This drug treats flowing-and-lodging, abscess and back effusion, going so far as to rescue a ruined illness and bring dead flesh back to life, to avert the trouble before it has sprouted and to pull out its root after it has already healed; it is greatly able to make the Qi move smoothly and the Blood run even. For Qi is yang and Blood is yin: if only the yang is regulated and the yin not harmonized, then the Qi is wasted and the congealed flesh will certainly not come alive, as with the likes of Ohyangyeongyo-tang; and if only the yin is harmonized and the yang not regulated, then the Blood flourishes while the Qi is weak, and the illness will certainly come back, as with the likes of Naebo-tang and Sipseon-san. This drug does both, and is truly a marvellous agent of the immortals. — Uirim 醫林 (Donguibogam · Ongjeo)

Clinical commentary 臨床

The richest leaf in the 流注骨疽 section and the one the chapter leans on hardest. Nine drugs at a flat 1 don each, decocted in half wine and half water, and three alternative names on top of the first — 追風通氣散, 通順散, 何首烏散. All three names say the same thing about it: it is a Qi-moving formula, and the phrase the leaf repeats twice, 大能順氣勻血, is its whole claim. The composition bears that out. Fleeceflower root, lindera, fennel fruit and bitter orange move and warm Qi; red peony and the Korean angelica move Blood; akebia drains; angelica dahurica is the one drug pointed at the sore itself. There is no heat-clearing in it at all, which is what separates it from almost everything else in the chapter, and no supplementation either. The closing passage explains why in so many words: formulas that regulate the yang alone, like 五香連翹湯, waste the Qi and leave dead flesh dead, while formulas that harmonize the yin alone, like 內補散 and 十宣散, let the illness return — so this one is offered as doing both, and the instruction to alternate it with 十宣散 with honeysuckle vine added to each is the practical form of that argument. The three modifications are worth keeping distinct. Blast-fried aconite for vacuity and rhubarb for repletion are a straight fork; arisaema and pinellia for abundant phlegm follow the leaf's reading of 流注 as a phlegm disease; cnidium, ephedra, scallion white, whole scorpion and pangolin scale are added when a swelling is hard and will not break. Pubescent angelica for 流注 gets its own argument, that once the vessels of the whole body are moving there can be no more lodging. 流注 — a swelling that appears in one place, settles, and reappears in another, with thin pus or clear fluid and little heat — covers migratory deep soft-tissue abscess and, read honestly, cold abscess of tuberculous origin; the chapter says plainly that 骨疽 is the ruined end of 流注. **The modifications bring in aconite, raw arisaema and pinellia, whole scorpion and pangolin scale. Aconite is cardiotoxic and has killed people in therapeutic doses; arisaema and pinellia are toxic unless properly processed; scorpion is toxic; pangolin scale is CITES Appendix I and prohibited in trade. The modified forms must not be prepared.** A deep abscess that migrates, or any swelling over bone, needs imaging, aspiration and culture — including for tuberculosis — rather than a Qi-moving decoction. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
