治卒中風痰塞ㆍ昏仆不省, 脉沉無熱者可服. 南星(生) 二錢, 川烏(生)ㆍ白附子(生) 各一錢, 木香半錢. 右剉, 作一貼, 入薑十五片, 水煎服. 《局方》 ○一名順氣散(烏ㆍ附皆炮用). 《得效》

Translation

Treats sudden wind stroke with phlegm blocking and collapse into stupor without awareness; it may be given where the pulse is sunken and there is no heat. Raw arisaema, 2 don; raw aconite kusnezoffii and raw typhonium, 1 don each; costus, half a don. Cut up, make one packet, add fifteen slices of ginger and decoct in water. Clinical commentary 臨床

The classic emergency formula for the cold, collapsed stroke, and the indication is unusually precise about when not to use it: the pulse must be sunken and there must be no heat. That is the tradition's way of selecting the hypotensive, cold, peripherally shut-down patient from the hypertensive flushed one, and giving raw aconite to the second would kill. Fifteen slices of ginger — the largest quantity in the chapter — is the only safeguard in the formula. The Deughyobang's variant, with both aconites blast-fried, is the version that was actually used by most hands.
