Donguibogam 東醫寶鑑

治虛勞咳嗽ㆍ唾膿血ㆍ心肺有熱. 黃芪(蜜灸) 一錢半, 白芍藥ㆍ生乾地黃ㆍ黃芩ㆍ麥門冬ㆍ人參ㆍ白茯苓ㆍ桔梗ㆍ知母各一錢, 甘草五分. 右剉, 作一貼, 入薑三片ㆍ竹葉三片ㆍ小麥一撮, 水煎服. 《丹心》

Translation

Treats cough of vacuity taxation with the spitting of pus and blood, where there is heat in the heart and lung. Honey-roasted astragalus, 1 don 5 fun; white peony, raw dried rehmannia, scutellaria, ophiopogon, ginseng, white poria, platycodon and anemarrhena, 1 don each; licorice, 5 fun. Cut up, make one packet, add three slices of ginger, three bamboo leaves and a pinch of wheat, and decoct in water. — Dansim 丹心 (Donguibogam · Haesu)

Clinical commentary 臨床

A supplementing formula with cooling drugs inside it rather than a cooling one: astragalus and ginseng lead, and anemarrhena, scutellaria and raw rehmannia are there to take the heat out without drying the patient further. Wheat and bamboo leaf at the end are the pair the Bogam uses for a restless, feverish patient, and they are the same two that carry Gamcho-somaek-daejo-tang. 唾膿血 with heat in the heart and lung describes a patient coughing purulent blood-streaked sputum while running a fever, and the formula is a slow one. **That presentation needs sputum culture, imaging and antituberculous or antibiotic treatment.** — Eui Young (Justin) Chung, L.Ac., Dipl.
