Donguibogam 東醫寶鑑

治久嗽ㆍ勞嗽及肺痿ㆍ唾血腥臭. 人參ㆍ桑白皮ㆍ地骨皮ㆍ知母ㆍ阿膠珠ㆍ鸎粟殼(蜜炒)ㆍ杏仁ㆍ桔梗ㆍ甘草各一錢. 右剉, 作一貼, 入大棗ㆍ烏梅各一枚, 水煎, 去滓, 入蜜一匙, 攪勻澄淸, 吸服. 《得效》 ○一名人參淸肺飮. 《入門》

Translation

Treats long-standing cough and taxation cough, and wilting lung with the spitting of blood that smells foul. Ginseng, mulberry root bark, lycium bark, anemarrhena, ass-hide glue in beads, honey-fried poppy capsule, apricot kernel, platycodon and licorice, 1 don each. Cut up, make one packet, add one jujube and one smoked plum, decoct in water, strain, stir in a spoonful of honey, let it settle clear, and sip it. ○ Also called Insamcheongpye-eum. — Seui Deukhyobang 得效 · Uihak Immun 入門 (Donguibogam · Haesu)

Clinical commentary 臨床

The formula for the worst of the chapter's chronic coughs: 唾血腥臭, blood-streaked sputum that smells foul, which is the classical description of a suppurating lung. A modern reader would separate pulmonary tuberculosis from bronchiectasis and lung abscess here; the Bogam calls the whole picture 肺痿, wilting lung, and treats it by nourishing what has been burnt away — ginseng, ass-hide glue, lycium bark and anemarrhena — rather than by attacking anything. **Contains poppy capsule, the dried capsule of the opium poppy. It is a controlled substance, and this must not be prepared.** Its presence tells what the formula was chiefly doing: 鸎粟殼 suppresses cough and relieves pain, and honey-frying it was the standard way of giving it for a cough. Codeine linctus is its direct descendant, and foul-smelling blood-streaked sputum needs sputum culture, imaging and antibiotic or antituberculous treatment. — Eui Young (Justin) Chung, L.Ac., Dipl.
