Rhinoceros horn and hawksbill shell are CITES Appendix I materials and cannot lawfully be obtained or used; water buffalo horn is the usual substitution for the first and the second is simply omitted. Cinnabar is mercuric sulphide and realgar arsenic sulphide; both are toxic. The Sokbang's practical note on the benzoin is the Bogam's own addition.

Translation

Treats sudden stroke by urgent wind with loss of speech and loss of consciousness, and wind striking the viscera with the spirit clouded and darkened. Rhinoceros horn, cinnabar, realgar, amber and hawksbill shell, 1 nyang each; ox gallstone, 5 don; borneol and musk, 2.5 don each; silver leaf, fifty leaves; gold leaf, fifty leaves, half used as a coating; benzoin, 1 nyang, strained in wine free of sand and grit and boiled to a paste. Powder the rest, put in the benzoin paste, mix evenly, and divide each nyang into forty pills; dissolve one pill in a ginseng decoction and take two or three times a day. Benzoin is hard by nature and slow to dissolve and is awkward in an emergency, so it is best to halve it and replace the rest with refined honey. — Gukbang 局方 · Sokbang 俗方 (Donguibogam · Pung)

Clinical commentary 臨床

One of the three great orifice-opening elixirs of the tradition, and the only one of them the chapter gives for stroke. The aromatics — musk, borneol, benzoin — are the active principle in its own terms: they are held to penetrate where nothing else reaches, and they are given with cooling minerals to answer the heat. The Korean note at the end is characteristic of this book and worth more than the formula: benzoin is hard and slow to dissolve, which is awkward in an emergency, so halve it and use honey. That is a pharmacist's remark about a real problem. **This must not be prepared** — it carries mercury and arsenic, and two of its materials are from protected species. — Eui Young (Justin) Chung, L.Ac., Dipl.
