Donguibogam 東醫寶鑑

治癩初起如紫雲癜風, 或發紫血疱瘡. 何首烏 四兩, 五加皮ㆍ白殭蠶ㆍ苦參ㆍ當歸 各二兩, 全蝎 一兩半, 惡實ㆍ羌活ㆍ獨活ㆍ白芷ㆍ細辛ㆍ生地黃ㆍ防己ㆍ黃連ㆍ赤芍藥ㆍ蟬退ㆍ防風ㆍ荊芥ㆍ蒼朮 各一兩. 右爲末, 酒糊和丸梧子大, 溫酒或米飮下七十丸. 《入門》

Translation

Treats leprosy at its onset, when it looks like purple-cloud dapple wind, or purple blood-blisters breaking out. Fleeceflower root 4 nyang; acanthopanax bark, silkworm, sophora root and Korean angelica 2 nyang each; whole scorpion 1 nyang 5 don; burdock seed, notopterygium, aralia, Dahurian angelica, asarum, raw rehmannia, sinomenium, coptis, red peony, cicada moulting, siler, schizonepeta and atractylodes lancea 1 nyang each. Powder the above, make pills with wine paste the size of a paulownia seed, and send down seventy in warm wine or rice water. — Immun 入門 (Donguibogam · Jechang)

Clinical commentary 臨床

The companion to Daemapung-hwan at the other early presentation: where that formula takes the anaesthetic patches of five colours, this one takes the purple ones, 紫雲癜風, the dapple that looks like a purple cloud, and the blood-blisters that follow. The 大風瘡 section also records 初起白屑紫雲如癜風然, that leprosy begins with white scurf and a purple cloud like dapple wind, so the three early formulas — Baekhwasa-hwan for the white scurf, this for the purple cloud, Daemapung-hwan for the numb patches — divide the onset between them. Fleeceflower root at four nyang heads it, with acanthopanax bark, silkworm, sophora root and Korean angelica behind, and then thirteen drugs at a nyang that are a wind-damp and cool-the-blood sweep. Scorpion and silkworm are the creature pair the chapter reaches for when a lesion will not shift. **Contains whole scorpion, which is toxic, sinomenium, which carries aristolochic acid and is nephrotoxic and carcinogenic, and asarum in quantity, which carries it too. This must not be prepared.** The lesions described are treatable — but with multidrug therapy, and the earlier the better, because nerve damage once established does not reverse. — Eui Young (Justin) Chung, L.Ac., Dipl.
