大麻風丸

The transcription writes 白歛, which the variant table folds to 白斂, the corpus form for ampelopsis root with eleven occurrences against two of 白歛; 白斂 is printed. 痒 is printed as the transcription and the corpus both have it, the corpus never using 癢. jogak appears twice, once for 皂角刺, the spine, among the weighed drugs and once for the geun of 皂角 pod made into the binding extract. Donguibogam 東醫寶鑑

治大麻風初起, 遍身瘡點五色, 不知痛痒麻木證. 苦參 一斤, 羌活ㆍ獨活ㆍ白芷ㆍ白斂ㆍ白蒺藜ㆍ天花粉ㆍ何首烏 各一兩三錢, 皂角刺ㆍ當歸 各二兩七錢. 右爲末, 別用皂角 一斤剉, 水煎五日, 去滓熬膏和丸, 溫酒下百丸. 《入門》

Translation

Treats great numbing wind at its onset, with sore-spots of five colours over the whole body and the sign of knowing neither pain nor itch, and numbness. Sophora root 1 geun; notopterygium, aralia, Dahurian angelica, ampelopsis root, tribulus, trichosanthes root and fleeceflower root 1 nyang 3 don each; gleditsia spine and Korean angelica 2 nyang 7 don each. Powder the above; separately take a geun of gleditsia pod, cut it up, boil it in water five days, strain off the dregs and reduce it to an extract to make the pills with, and send down a hundred in warm wine. — Immun 入門 (Donguibogam · Jechang)

Clinical commentary 臨床

The chapter's formula for leprosy caught early, and the indication is the most clinically exact in the section: 遍身瘡點五色, 不知痛痒麻木 — patches of several colours over the body that have no sensation of pain or itch, with numbness. That is hypopigmented and erythematous anaesthetic macules with peripheral nerve involvement, and it is how early Hansen's disease actually presents. The 五死 passage above sets 皮死麻木不仁 as the first of the five deaths for the same reason. A geun of sophora root and a geun of gleditsia pod make the formula's bulk, with the two 活, Dahurian angelica, ampelopsis, tribulus, trichosanthes root and fleeceflower root at a nyang three don each, and gleditsia spine and Korean angelica raised to two nyang seven. A hundred pills to a dose is a large daily intake of sophora. Anaesthetic skin is the thing to act on in this picture, and acting on it now means nerve-function testing and multidrug therapy begun at once, because the nerve damage that follows is what disables.
