Boost the Qi and Nourish the Constructive Decoction

Ikgiyangyeong-tang

한글 익기양영탕  ·  Pinyin Yi Qi Yang Rong Tang

益氣養榮湯

The collation line 脯 : 晡(初刊本) sits at the page boundary after the 連翹散堅湯 leaf, before the [033b] marker, but the word it annotates is the 日脯 of this leaf at 033a; 脯 occurs nowhere in 連翹散堅湯. The transcription's reading 日脯 is printed above, the 初刊本 reading being 日晡, the late afternoon, which is the sense in either case. The transcription prints 欝 for 鬱, 歛 for 斂 and 鍳 for 鑑; the corpus forms are printed above. 灸 is left as the transcription and the corpus both print it for the mix-frying of licorice.

Translation

Treats feeling pent up and held in the breast, scrofula with flowing-pour swellings, fever coming on in the late afternoon, or a sore that has broken and will not close. Astragalus 1.5 don; ginseng and white atractylodes 1 don each; Korean angelica (wine-washed), cnidium, white peony (wine-fried), raw rehmannia, tangerine peel, cyperus and fritillary 7 fun each; bupleurum, platycodon, lycium bark and licorice (mix-fried) 5 fun each. Cut up, make one packet, decoct in water and take, twice a day. — Uigam 醫鑑 (Donguibogam · Jechang)

Clinical commentary 臨床

Eight-Treasure territory with the surgical additions: 黃芪 leading, 四君 minus poria, 四物 with raw rehmannia instead of cooked, and then 香附子, 貝母, 柴胡, 桔梗 and 地骨皮 laid over the top. The base builds Qi and Blood, and the five additions answer the leaf's first clause rather than its last — 懷抱抑鬱, feeling pent up and held in the breast, is named as the cause, so cyperus and bupleurum are there to move what is bound, fritillary and platycodon to soften and lift the mass, and lycium bark for the late-afternoon fever. That fever is the diagnostic detail worth keeping. 日脯發熱 with a node that has broken and will not close, in a patient who is wasting, is the clinical picture of active tuberculous lymphadenitis — an evening temperature, a cold abscess tracking down the neck, and a sinus that drains thin pus for months. The Uigam's instinct to stop cooling and start supplementing is sound as supportive care and was probably the best of the chapter's options, but it reaches nothing: a tuberculous sinus closes only when the mycobacterium is treated. — Eui Young (Justin) Chung, L.Ac., Dipl.
