Donguibogam 東醫寶鑑

治馬刀ㆍ瘰癧流注, 遍體作核作瘡. 柴胡 一錢半, 草龍膽(酒炒四次)ㆍ土瓜根(酒炒) 各一錢二分, 黃芩(酒炒三次) 一錢, 當歸梢ㆍ生黃芩ㆍ蓬朮(酒炒)ㆍ三稜(酒炒)ㆍ連翹ㆍ白芍藥(酒炒) 各七分, 灸甘草 五分, 黃連(酒炒)ㆍ蒼朮 各四分. 右剉, 作一貼, 水浸半日, 乃煎, 去枕仰臥, 一口作十次嚥之, 留一口送下丸藥. 另取一劑爲細末, 蜜丸菉豆大, 每百丸, 以煎藥水送下. 更以<a href='/formulas/yongcheon-san-jechang'>龍泉散</a>塗之. 《正傳》

Translation

Treats the 馬刀 sore and scrofula with flowing-pour swellings, nodes and sores forming over the whole body. Bupleurum 1.5 don; gentian (wine-fried four times) and snake gourd root (wine-fried) 1 don 2 fun each; scutellaria (wine-fried three times) 1 don; the fine tail of Korean angelica root, raw scutellaria, curcuma (wine-fried), sparganium (wine-fried), forsythia and white peony (wine-fried) 7 fun each; mix-fried licorice 5 fun; coptis (wine-fried) and atractylodes lancea 4 fun each. Cut up, make one packet, soak it in water half a day, then decoct. Take the pillow away and lie flat on the back, making ten swallows of one mouthful, and keep one mouthful back to send down the pills. Take a second dose, make it into a fine powder and honey pills the size of a mung bean, and send down a hundred pills each time with the decoction. Apply Yongcheon-san on the outside as well. — Jeongjeon 正傳 (Donguibogam · Jechang)

Clinical commentary 臨床

Dong-won's 散腫潰堅湯 rebuilt by the Jeongjeon for the generalised case — bupleurum raised to the lead at a don and a half, gentian wine-fried four times, scutellaria entered twice over at two different strengths, and the hardness-breaking pair 三稜 and 蓬朮 kept. Almost every bitter in it is wine-fried, which is the Yi Shui convention for carrying a cooling drug up to the head and neck rather than letting it drain downward. The administration is as deliberate as the composition. The packet soaks half a day, then the patient lies flat with the pillow removed and takes a single mouthful in ten separate swallows, so the decoction dwells on the throat and the sides of the neck instead of going straight down; the last mouthful carries a hundred honey pills made from the same formula. Then Yongcheon-san goes on the outside. It is a three-way assault on one lesion. 遍體作核作瘡 — nodes and sores over the whole body — is generalised tuberculous lymphadenitis, often with cold abscesses tracking along fascia, and the 流注 of the name is exactly that tracking.
