Toxin-Resolving Wash (Abscess Formula)

Haedok-tang

한글 해독탕  ·  Pinyin Jie Du Tang

解毒湯

Donguibogam carries two formulas called 解毒湯. This is the Ongjeo one, an external steam and wash; the Chi chapter formula of the same name is registered as haedok-tang-chi. 黃栢 and 葱白 are printed as the transcription has them, since the corpus prints those forms. The scallion stalks, the jujubes, the pig's trotter and the 三升 of water are count and volume items and carry no gram figure. The alternative name given from 得效, 水師精明, is printed exactly as the transcription has it; it is an unusual name, the base text offers no gloss for it, and it may be corrupt.

Translation

Treats abscesses whether unbroken or already broken — both are washed with it. Where pus has formed and the sore is ulcerated and rotting, washing it clean is the first necessity, and only then is powder or plaster applied. Phellodendron, lycopus, licorice, schizonepeta, red peony, rhubarb, angelica dahurica, Korean angelica and pubescent angelica, 2 don each. Cut them up small, add five stalks of scallion white and five jujubes and 3 seung of water, decoct them together, strain off the dregs, and while it is still warm steam and wash the wound with it. If the sore is ulcerated and rotting, boil a pig's trotter in with it, which spares the patient the dry pain. ○ One name for it is Susa Jeongmyeong. — Deukhyo 得效 (Donguibogam · Ongjeo)

Clinical commentary 臨床

Nine drugs at an even two don, with scallion white and jujube, boiled in three seung of water and used warm — first as a steam held over the wound and then as a wash. 黃栢 and 大黃 carry the heat and the toxin, 澤蘭ㆍ赤芍藥ㆍ當歸 the blood, 荊芥ㆍ白芷ㆍ獨活 the surface, and 甘草 softens the whole. It is the one wash in the group given for both stages of an abscess, before it breaks and after. Nothing here is swallowed. The order the leaf insists on is what matters most in it: wash clean first — 最要洗淨 — and apply powder or plaster only afterwards. That is still the sequence for dressing a suppurating wound. The refinement about the pig's trotter is a genuine observation: gelatin in the broth keeps the wash from pulling at raw tissue, which is why 可免乾痛, it spares the dry pain, and why bland viscous irrigants are easier on a granulating surface than thin ones. The modern form of all this is irrigation with sterile saline. A warm plant decoction standing in a kitchen is not sterile, and poured into an open abscess it can introduce more than it removes. **A pig's trotter is a raw-animal material and carries an infection risk on broken skin.** An unbroken abscess, which this leaf also treats, is not helped by washing at all; it needs drainage. — Eui Young (Justin) Chung, L.Ac., Dipl.
