Angelica and Notopterygium Decoction

Danggwiganghwal-tang

한글 당귀강활탕  ·  Pinyin Dang Gui Qiang Huo Tang

當歸羌活湯

水浸半日 — the cut packet stands in water for half a day before it goes on the fire, which is unusual in the chapter and belongs to the wind-drugs in it. The 木香ㆍ檳榔 at the foot are not decocted: they are powdered and stirred into the clear part of the strained liquid, so the leaf is twelve drugs boiled and two taken as powder; the chapter has a 木香檳榔散 of its own. The transcription prints 癕, 𤍠 and 欝; the corpus prints 癰, 熱 and 鬱. 梔子 is the form the corpus uses for the gardenia.

Translation

Treats the five eruptions of abscess, and is most suitable where rich food and fat meat have banked up heat. Korean angelica, scutellaria and coptis, all three wine-processed, 1 don 5 fun each; wine-processed phellodendron, forsythia, siler, notopterygium, gardenia and licorice, 7 fun each; aralia and ligusticum root, 5 fun each; alisma, 3 fun. Cut the above, make one packet, steep it in water for half a day, add a spoonful of wine, decoct and take hot. Twice a day, six doses finished in three days. Then take 1 don each of powdered costus root and areca seed, stirred into the clear part of the decoction. — Uihak Immun 入門 (Donguibogam · Ongjeo)

Clinical commentary 臨床

This is a Dongwon-style construction: three wine-processed bitters at the centre to drain the banked heat, four wind-drugs — siler, notopterygium, aralia, ligusticum root — to lift it out through the surface, and alisma at 3 fun to let it down through the urine. Korean angelica is there for the blood, and gives the formula half its name; notopterygium gives the other half and marks which of the two directions the author thought mattered. 膏粱熱鬱 is the indication worth reading plainly. 膏粱 is fat meat and fine grain, the diet of the well-off, and the chapter's claim is that abscesses in such a patient come of heat that the diet has banked up rather than of an outside toxin. That is a recognisable clinical picture: recurrent furuncles and carbuncles cluster in people with poor glycaemic control, and a first abscess in an adult is a reason to check for diabetes. The dosing is specified unusually tightly — twice a day, six doses finished in three days — and then the costus and areca powder, which are moving drugs, go in at the end. A three-day course with a stop built into it is a more careful instruction than most leaves in this chapter carry. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
