Translation

Treats an abscess and deep-rooted abscess at first onset, with aversion to cold and vigorous heat, and in a severe case headache and cramping tension, so that it looks like cold damage. In the first four or five days, two or three doses; in a mild case it disperses of itself. If it does not disperse, Seonbang Hwalmyeong-eum should be taken. Notopterygium, aralia, bupleurum, peucedanum, platycodon, cnidium, red poria, honeysuckle, bitter orange, forsythia, siler, schizonepeta, mint and licorice, 7 fun each. Cut the above, make one packet, add three slices of ginger, and decoct in water and take. — Uigam 醫鑑 (Donguibogam · Ongjeo)

Clinical commentary 臨床

敗毒散 with honeysuckle and forsythia put in front of it. The base is 荊防敗毒散 — notopterygium, aralia, bupleurum, peucedanum, platycodon, cnidium, red poria, bitter orange, siler, schizonepeta, mint and licorice — and the two additions are what make it a sores formula rather than an exterior-releasing one. The indication explains why an exterior formula is here at all: an abscess at first onset presents with chills, high fever, headache and body tension, 狀似傷寒, looking exactly like cold damage, and the chapter is warning that the two are mistaken for one another. The instruction is unusually precise and unusually modest — two or three doses inside the first four or five days, and the leaf itself says that if the swelling does not go down you should move on to 仙方活命飮. That is a time-limited trial with a stated next step, which is good clinical writing. The modern reading of 狀似傷寒 is the one worth keeping: fever with rigors and myalgia may be the first sign of a deep soft-tissue infection, and the focus is easy to miss early. Fever with any area of spreading redness, or with pain out of proportion to what is visible, needs examination for necrotizing infection, not a time-limited trial. — Eui Young (Justin) Chung, L.Ac., Dipl.
