Donguibogam 東醫寶鑑

治癰疽ㆍ發背ㆍ乳癰, 一切無名腫毒, 未成立消, 已成立潰. 金銀花ㆍ天花粉各一錢二分, 防風ㆍ黃芩ㆍ甘草節ㆍ白芍藥ㆍ赤茯苓ㆍ貝母ㆍ連翹ㆍ白芷各一錢, 半夏七分, 乳香ㆍ沒藥各五分. 右剉, 作一貼, 酒水相半煎服. 《回春》

Translation

Treats abscess and deep-rooted abscess, back effusion and breast abscess, and every unnamed swelling toxin: if it has not yet formed it disperses it at once, and if it has formed it ruptures it at once. Honeysuckle and trichosanthes root, 1 don 2 fun each; siler, scutellaria, licorice nodes, white peony, red poria, fritillaria, forsythia and dahurian angelica, 1 don each; pinellia 7 fun; frankincense and myrrh, 5 fun each. Cut the above, make one packet, and decoct in half wine and half water and take. — Hoechun 回春 (Donguibogam · Ongjeo)

Clinical commentary 臨床

Honeysuckle and trichosanthes root at the head, with forsythia, fritillaria and scutellaria behind them: the toxin-resolving core that runs through the whole chapter. White peony and dahurian angelica move into the sore, frankincense and myrrh take the pain, and pinellia and red poria drain the damp that collects under a swelling. The half-wine decoction is there to carry the drugs to the surface. The claim in the indication is the useful part, and it is a claim about timing rather than about one disease: before a head has formed the formula is meant to make the swelling go down, and after it has formed it is meant to bring it to rupture. That is the same decision a surgeon still makes between antibiotics and incision, and it is why this leaf stands beside the 托裏 formulas rather than among the draining ones. **Contains raw pinellia, which is toxic unless properly processed.** An abscess with spreading redness, fever or rapidly worsening pain needs antibiotics and, where pus has collected, drainage. — Eui Young (Justin) Chung, L.Ac., Dipl.
