Another Formula for Drawing Out Calomel

Ilbang

한글 일방(취경분법)  ·  Pinyin Yi Fang

一方

Given in the source as 一方, an unnamed formula standing in the 取輕粉法 section of the 天疱瘡 chapter, after the 仙遺粮 leaf, and entered here under an identifier taken from that section. It has nothing to do with the three unnamed 一方 entries of the 黃疸 chapter (chihyeok-bang-1 and following), or with the 肺癰 and 疔疽 ones of the abscesses chapter (pyeongong-ilbang, jeongjeo-ilbang). The transcription writes 觧 for 解; the corpus form is printed. 鍾 is left as the transcription has it, the corpus printing 鍾 at all thirty-two places and 鐘 at none. 葦根, reed root, has no headword of its own and is entered under 蘆根, the same drug of the same plant; the transcription's 葦根 is kept in the preparation. 鴨, a duck with no further qualification, is entered under 白鴨, the herb list's headword for the bird. The water the duck is given and the pounding are method, not ingredients.

Translation

Treats pox sores and water-blister sores of every kind. Sesame oil, 2 geun; add one cup of water, boil until white smoke rises, and store it. For each dose take one cup of yellow rice wine, stir in one cup of the oil, and drink it warm, three times a day; when the whole lot has been drunk the cure is complete. ○ Another method. One duck, starved for two days and given nothing but plain water to drink. Take calomel 1 nyang and cooked rice 4 nyang, mix them evenly and feed them to the duck; when it has eaten the lot, pound reed root, steep it in water and let the duck drink it, which resolves the calomel toxin. Wait until all the duck's feathers have dropped, then boil the duck and eat it. — Jonghaeng 種杏 (Donguibogam · Jechang)

Clinical commentary 臨床

Two methods, and the second is the reason the leaf sits where it does. The 取輕粉法 section is not about the pox at all — it is about getting the calomel back out of a patient who has already been treated with it, and this leaf gives a way of doing that which is hard to read as anything but magical thinking. A duck is starved for two days, fed a nyang of calomel in four nyang of rice, watered with pounded reed root to draw the poison off, and when its feathers have all fallen out it is boiled and eaten. The feather loss is taken as proof that the mercury has gone into the bird; eating the bird is then supposed to be safe, and somehow curative. The first method is milder and more ordinary: two geun of sesame oil boiled down until it smokes white, then a cup of it drunk warm in yellow wine three times a day until the batch is finished. Oil taken internally in quantity is a purgative and an emollient, and against the deep ulcerating sores of late syphilis it does nothing. **Contains calomel, mercurous chloride. Mercury was the standard treatment for syphilis for four centuries and it poisons: salivation, loosened teeth, tremor, kidney failure, neurological damage. The chapter itself records this at the head of the 天疱瘡 section — 或誤服水銀輕粉而成風堆腫爛流膿出汁, swelling, breakdown and discharge following on mercury wrongly taken — so the Bogam knew the treatment was producing the disease it then had to treat. This must not be prepared.** The duck method would concentrate the mercury in the bird's tissues, not remove it; eating the duck would be a second poisoning. Syphilis is curable with penicillin. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
