Donguibogam 東醫寶鑑

一少婦膺間潰一竅, 口中所咳膿血與竅相應而出. 宜大補氣血, 以參芪ㆍ當歸加退熱排膿藥多服, 兼取雲母膏作丸, 甘桔湯呑下而愈. 《丹心》

Translation

A young woman had an aperture open in the upper chest, and the pus and blood she coughed up from her mouth came out through the aperture in answer to the cough. The Qi and Blood should be greatly supplemented: give ginseng, astragalus and Korean angelica with heat-withdrawing and pus-expelling drugs added, in large doses, and alongside them take Unmo-go made up into pills, swallowed down with Gamgil-tang. Clinical commentary 臨床

A case rather than a prescription. A young woman's chest had suppurated through to the outside, and what she coughed up came out of the opening — which is why Dansim's answer is not to cool harder but to supplement: ginseng, astragalus and Korean angelica in large doses, with the heat-withdrawing and pus-expelling drugs added on top of that base. This is the 托裏 reasoning in its clearest form, that a suppuration which will not close needs the Qi and Blood built up under it. A collection that has eaten its way through the chest wall and continues to drain with the cough is empyema necessitatis with a fistula, and it is a serious, chronic thing. 肺癰 read this way — purulent bloody expectoration with a foul breath — is lung abscess, bronchiectasis or empyema, and the Bogam is describing the late, draining end of it. **A discharging chest sinus needs surgical assessment, imaging and antibiotics.
