Thousand Gold Lump-Dispersing Pill

Cheongeumsobyeok-hwan

한글 천금소벽환  ·  Pinyin Qian Jin Xiao Pi Wan

千金消癖丸

The twenty largest formula in this round and the only one in the 積癖 section with no mineral and no violent purgative in it. 山査肉 is kept as the transcription prints it and resolves to 山査; the corpus prints 山査肉 in forty places. The asafoetida is both an ingredient and the binder, ground with water into the flour paste, and is entered once.

Clinical commentary 臨床

Twenty drugs in four weight bands. Prince's feather seed, leaven and malt lead at 4 don; the 四君子 core of ginseng, atractylodes and poria follows at 3 don; then the worm and bitter drugs — Rangoon creeper fruit, picrorhiza, hawthorn, cyperus, sparganium, zedoary — at 2 don; and a long tail of aloe, asafoetida, indigo, costus, areca, magnolia bark, tangerine peel and liquorice at 1 don. It is a composite: a supplementing formula, a digestive formula, a worm formula and a mass-breaking formula written into one pill. For what 積癖 actually is and why a persistent abdominal mass in a child is an imaging question today, see 進食丸 (jinsik-hwan), where the differential for this batch is set out in full. Two things in it do work. **使君子 is Rangoon creeper fruit and 檳榔 is areca, and both are genuine anthelmintics** — 使君子 is still used against ascaris and 檳榔 against tapeworm — so a child whose abdominal distension came from a heavy roundworm load might really improve, and the chapter's own 1.38.4 蛔疳 is that child. A single dose of albendazole or mebendazole does the same thing now, with a known dose; an effective drug with no stated dose is still a hazard, and enough 使君子 causes hiccup, vomiting and diarrhoea. **Contains aloe (蘆薈), a stimulant laxative. The patient is a child, the chapter gives no dose by weight anywhere, and in a child already losing fluid to 疳瀉 or 吐瀉 a stimulant laxative is dangerous on its own account.** 胡黃連 and 靑黛 are bitters, and bitters are not food; the one honest limit of this formula is that the child it is written for is usually malnourished, and nothing in twenty drugs supplies a calorie. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
