Clinical commentary 臨床

The broadest of the three decoctions in this section. Forsythia heads a venting-and-clearing group — burdock seed, schizonepeta, ledebouriella — while red peony, tangkuei, cnidium and dried raw rehmannia work on the blood, and gardenia, dianthus and akebia with the rush pith drain heat downward through the urine. Ophiopogon and trichosanthes root replace fluid, liquorice harmonises. Nothing is given more weight than anything else, which is characteristic of the Deukhyo's generalist formulas. Two indications are named. 惡瘡遍身痛痒 is the section's subject, and 血風瘡, the blood-wind sore, is the tradition's name for a chronic, intensely itching eruption that scabs and recurs — close to what would now be called chronic eczema or prurigo, where scratching maintains the lesion. This is a reasonable formula on its own terms and holds nothing dangerous. It is worth saying plainly that for a chronic itching eruption the one intervention that reliably helps is breaking the itch-scratch cycle, and that a sore running pus needs its infection treated rather than vented. — Eui Young (Justin) Chung, L.Ac., Dipl.
