Level the Blood Drink

Pyeonghyeol-eum

한글 평혈음  ·  Pinyin Ping Xue Yin

平血飮

The leaf gives no composition and no weights of its own: it names Seungmagalgeun-tang as the base, two drugs added to it, and four more put in when it is boiled together with Insampaedok-san. Only those six are entered, without grams, as the corpus does for other 卽…加… heads. 蟬殼 is entered under 蟬退. The 細註 方見寒門 stands immediately after 升麻葛根湯 and annotates that formula; the section head above carries the same cross-reference on 人參敗毒散, but this leaf does not.

Translation

Treats sores over the whole body that run pus and blood, with pain and itching. This is Seungmagalgeun-tang — the formula is given in the Cold chapter — with gastrodia and cicada slough added. Cut the above up, mix it together with Insampaedok-san, add fresh ginger, mint, raw rehmannia and ophiopogon, and decoct and take. — Seui Deukhyobang 得效 (Donguibogam · Jechang)

Clinical commentary 臨床

Two formulas run together. Seungmagalgeun-tang — cimicifuga, pueraria, white peony and liquorice — is the standard exterior-releasing base for a rash that is coming out, and Insampaedok-san is the wind-damp exterior formula of the Cold chapter; the leaf stacks them and adds gastrodia and cicada slough for the itching, then ginger and mint at the boil to keep the surface open and raw rehmannia and ophiopogon to cool and moisten underneath. The indication is the section's own: 遍身出膿血, sores over the whole body running pus and blood. That is a generalised, weeping, secondarily infected eruption, and the logic of the prescription is to push outward and cool at once rather than to treat the skin topically. Nothing here reaches a staphylococcal or streptococcal infection of broken skin, which is what widespread pustular sores usually are. The base formulas are mild and the drug list holds nothing hazardous, but a generalised pustular eruption with systemic signs needs an antibiotic and a diagnosis, not a decoction. — Eui Young (Justin) Chung, L.Ac., Dipl.
