又方

Given in the source as 又方, an unnamed head standing at the end of the 癭瘤 section, and entered here under a descriptive identifier taken from the section. It has nothing to do with the other unnamed 又方 head of this chapter in the 結核 section, registered as gyeolhaek-ubang, nor with the 一方 entries of other chapters. The base text gathers seven remedies from three sources under the one head and they are kept together. A collation line, 潰 : 漬(初刊本), stands on its own line after the 疳瘻 section head two pages further on, past [039a] and the 灸法 and 疳瘻 heads; the word it annotates is very probably this leaf's 酒潰 in the last clause, which the first edition reads 酒漬, steeped in wine — the same operation that line 2 of this leaf writes correctly as 酒三升漬數日 — rather than the 諸瘻之潰漏也 of the section head it sits beneath, where 漬 makes no sense. The transcription's 潰 is printed above as the 底本 rule requires and the variant recorded here. 醎 is printed as the corpus majority has it. 松蘿, the lichen, is entered as an ingredient. 羊靨 and 鹿靨, the thyroid glands of sheep and deer, have no entries in this corpus's herb list and are not entered as ingredients; they are named here. 灸 is left as the transcription prints it.

Clinical commentary 臨床

凡海中菜皆療癭瘤結氣 — all sea vegetables treat goitre, tumour and bound qi. That single clause, taken from the 本草, is the most important sentence in the 癭瘤 section and one of the few places in the whole chapter where a traditional generalisation is simply correct. Endemic goitre is iodine deficiency; marine algae are the richest dietary source of iodine; and the 本草 had worked out from observation that the whole class of sea vegetables, not any one of them, answers for a swollen neck. The two seaweed remedies that follow it are a wine infusion of sargassum sipped slowly and a sargassum-and-kelp honey pill held in the mouth — both low, continuous doses, which is how iodine should be given. The last remedy is the other half of the modern picture. 羊靨ㆍ鹿靨, the thyroid glands of sheep and deer, steeped in wine and roasted, held in the mouth and the juice swallowed, better in seven days: that is thyroid extract, and in a hypothyroid goitre it would do what the leaf says in about that time. The first remedy is the one that does not work, and it is worth saying so. Needle filings left in the household water jar for ten days at a stretch, for half a year, puts iron into the drinking water, not iodine, and iron does nothing for a goitre. What it would do is correct an iron-deficiency anaemia, which in a worn-out patient with a neck swelling might well have looked like improvement. Selfheal and lichen are the chapter's plant drugs for a neck lump and have no such mechanism, and the spider in wine is sympathetic magic. **Sheep and deer thyroid contain thyroid hormone in an unmeasured dose, which causes thyrotoxicosis, and are raw animal glandular tissue; a geun of sargassum steeped in wine is an iodine load no one can estimate, and large iodine loads can tip a nodular goitre into hyperthyroidism or shut a thyroid down. This must not be prepared.** A goitre is assessed with thyroid function tests and an ultrasound; iodine deficiency is corrected with iodised salt at dietary dose, and a failed thyroid with levothyroxine titrated to blood tests. — Eui Young (Justin) Chung, L.Ac., Dipl.
