Lick the Palm Powder

Jijang-san

한글 지장산  ·  Pinyin Shi Zhang San

䑛掌散

The head is printed 䑛掌散 with the variant graph of 舐, to lick; the corpus has no other instance of either graph, so the leaf's own form is kept and the name romanised from the 舐 reading. The transcription writes 靑𥂁 with the variant graph for 鹽, given above as 鹽. 蓬朮 is registered under 蓬朮 by way of the 莪朮 alias. The 丹心 一方 appended to the leaf is a second composition under the same head; its drugs are entered at 等分 with no weight, marked in the preparation, as the corpus does for gathered leaves.

Translation

Treats goitre. Sargassum and coptis, 1 nyang each. Powder the above, take a little and put it in the palm of the hand, lick it from time to time, and swallow the saliva. When the goitre has gone down by two thirds, stop the drug. ○ Another formula: sargassum, sea tangle, kelp, zedoary and green salt in equal parts, powdered, made into honey pills the size of a bullet, held in the mouth to dissolve and swallowed, three times a day. — Jeongjeon 正傳 · Dansim 丹心 (Donguibogam · Jechang)

Clinical commentary 臨床

The delivery is the prescription. Sargassum and coptis, powdered, a pinch at a time in the hollow of the hand, licked off through the day and the saliva swallowed — which gives a small continuous dose over weeks instead of a measured one twice a day. For iodine that is exactly the right way to give it. Sargassum is one of the richest iodine sources known, endemic goitre is iodine deficiency, and the gland takes up iodide steadily rather than in boluses. 如消三分之二止藥 is the clause to notice: stop the drug when the swelling is down by two thirds, not when it is gone. That is a stopping rule, and it is written into the prescription rather than left to judgement. It also happens to be prudent for the reason the 正傳 could not have known — pushing iodine into a nodular goitre past the point of benefit can tip it into hyperthyroidism, and a residual third of the swelling is a cheap price for not finding out. The 丹心 formula appended to the leaf is the same idea in pill form: three seaweeds, zedoary to break up hardness, and green salt, held in the mouth three times a day. **Coptis at 1 nyang in a powder licked over days is a large cumulative dose of a bitter alkaloid drug, and the iodine load of a seaweed powder taken continuously is not measurable. A goitre needs thyroid function tested and an ultrasound before anything is given; iodised salt, at dietary dose, is what replaces iodine now.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
