Ginseng Goitre-Dissolving Elixir

Insamhwayeong-dan

한글 인삼화영단  ·  Pinyin Ren Shen Hua Ying Dan

人參化癭丹

The name promises ginseng and the drug list does not contain it; 人參 appears nowhere in the leaf and no collation note stands at this page. The discrepancy is printed as transcribed and flagged here. 猪靨 and 羊靨, the thyroid glands of the pig and the sheep, have no entries in this corpus's herb list and are not entered as ingredients; they are named here and the drug list is otherwise complete. The interlinear 卽猪羊外腎乃囊中之卵也 is a substance gloss rather than a cross-reference and is set in parentheses, as the corpus does for interlinear glosses of this kind — and it is mistaken: it identifies 靨 as the testicle, whereas the 神效開結散 leaf two heads later describes the same organ accurately as 生猪項下喉嚨系一枚如棗大微扁色紅者, one body below the pig's neck attached to the throat, jujube-sized, slightly flattened and red, which is the thyroid gland. The transcription writes 醎 and 醫鍳; 醎 is printed as the corpus majority has it and 醫鍳 is given as 醫鑑. The 各十枚 of the glands is a count and carries no weight.

Translation

Treats goitre and tumour. Sea tangle, sargassum, clam-shell powder and kelp — the four above all dried over heat — with stir-fried alisma and forsythia, 1 nyang each; pig thyroid and sheep thyroid (that is, the outer kidneys of the pig and the sheep, the eggs within the sac), ten of each, sliced and dried over heat. Powder the above, make honey pills the size of a gorgon nut, and after food hold one pill in the mouth and let it dissolve, three times a day. ○ Goitre and tumour are mostly treated with kelp, sargassum and sea tangle, salty drugs that can soften hardness. — Gangmok 綱目 · Uigam 醫鑑 (Donguibogam · Jechang)

Clinical commentary 臨床

Three of the first four drugs are seaweeds, dried over heat and taken as a honey pill held in the mouth three times a day, and ten pig and ten sheep thyroid glands are ground in with them. This is the one prescription in the chapter where the traditional drug and the modern mechanism are the same thing. Endemic goitre is iodine deficiency; sargassum, kelp and sea tangle are the richest dietary sources of iodine there are; and the thyroid gland of a food animal contains thyroid hormone, which is the drug still used for hypothyroid goitre. A pill of seaweed and thyroid, held in the mouth and swallowed slowly after meals, is iodine replacement with thyroid extract added — and the 綱目 arrived at it in the sixteenth century by the reasoning quoted at the foot of the leaf: 醎能軟堅, salty drugs soften what is hard. The 靨 is the point on which the leaf is both right and wrong. The gloss the Bogam carries says the glands are the animals' testicles, which they are not; the accurate description of the same organ stands two heads further on, in 神效開結散. Whoever wrote the gloss had never seen the dissection, and whoever wrote 神效開結散 had. The prescription works through the organ the gloss misnames. None of which makes it safe to prepare. **Thyroid glands of pig and sheep contain thyroid hormone in an unmeasured dose, and an unmeasured dose causes thyrotoxicosis — palpitation, tremor, weight loss, atrial fibrillation and, in the old thyroid-extract era, deaths. Large iodine loads can also tip a nodular goitre into hyperthyroidism or shut a thyroid down. Raw animal glandular tissue carries an infection risk besides. This must not be prepared.** A goitre today is assessed with thyroid function tests, antibodies and an ultrasound, and treated with iodised salt where deficiency is the cause and with levothyroxine at a measured dose where the gland has failed. — Eui Young (Justin) Chung, L.Ac., Dipl.
