Donguibogam 東醫寶鑑

治心癰, 退潮, 止渴, 解熱, 能內消. 木通ㆍ瞿麥ㆍ荊芥ㆍ薄荷ㆍ白芷ㆍ天花粉ㆍ赤芍藥ㆍ麥門冬ㆍ生乾地黃ㆍ梔子ㆍ車前子ㆍ連翹ㆍ甘草 各八分. 右剉, 作一貼, 入燈心ㆍ竹葉, 水煎服. ○一名引兵先鋒. 《得效》

Translation

Treats heart abscess, withdrawing the tidal fever, stopping thirst, resolving heat, and able to disperse it inwardly. Akebia, dianthus, schizonepeta, mint, angelica dahurica, trichosanthes root, red peony, ophiopogon, dried rehmannia, gardenia, plantago seed, forsythia and licorice, 8 fun each. Cut up, make one packet, add juncus and bamboo leaf, decoct in water and take. ○ Another name: Vanguard of the Marching Troops. — Seui Deukhyobang 得效 (Donguibogam · Ongjeo)

Clinical commentary 臨床

Despite the name, very little of this cools the Blood. The body of it is a urine-freeing battery — akebia, dianthus, plantago seed, gardenia, juncus and bamboo leaf — with forsythia and mint at the surface and red peony, dried rehmannia, ophiopogon and trichosanthes root holding the fluids. The strategy is to take heat out of the Heart by the bladder, which is the standard Bogam move for 心火, and 退潮 and 止渴 are the two signs it claims to settle. The Deukhyo gives it a second name, 引兵先鋒, Vanguard of the Marching Troops — it goes in first, before Gamisipgi-san is brought up behind it. The chapter's 心癰 is a swelling at 巨闕, between the chest and the breast, said to look like a honeycomb and to kill in seven days if it reaches the abdomen. That is a vivid description but it does not map onto a modern diagnosis, and it should not be forced to: 心癰 is best left as the chapter's own category rather than translated into an entity. **A hot, spreading swelling of the chest wall with fever is an infection and needs assessment and antibiotics.** — Eui Young (Justin) Chung, L.Ac., Dipl.
