Ox Gallstone Dragon-Embracing Pill

Uhwangporyong-hwan

한글 우황포룡환  ·  Pinyin Niu Huang Bao Long Wan

牛黃抱龍丸

The transcription writes 䧺 for 雄, 真 for 眞, 為 for 爲, 煑 for 煮 and 鍳 for 鑑; the standard forms are printed. A collation note sits two leaves below, at 023a, reading 䧺 : 雄(First Edition); the character it annotates is not in the leaf it sits inside but in this leaf and in 探生散, and it is a plain glyph variant already covered by the corpus normalisation. A second collation note, 真 : 眞(First Edition), sits inside the following leaf 保命丹 at 022a and annotates the 眞珠 of this one. The closing clause compares the formula with 鎭驚丸 of 1.29 急驚風, which is a leaf of this same chapter not yet registered; the name is left as plain text and the intended id is carried in `related`.

Arisaema processed in ox bile, 1 nyang; bamboo silica, 5 don; realgar and cinnabar, 2 don 5 fun each; musk, pearl and amber, 1 don each; ox gallstone, 5 fun; gold leaf, ten leaves. Powder the above finely, bind with a licorice paste boiled in water, and roll into pills the size of a water-chestnut seed, coated with gold leaf. A three-year-old takes one pill, a five-year-old two, a ten-year-old three to five, dissolved down in a mint decoction. — Uigam 醫鑑 ○This formula is the same as Jingyeong-hwan 鎭驚丸 above, differing only in the weights. — Uigam 醫鑑 (Donguibogam · Soa)

Clinical commentary 臨床

The chapter's 抱龍丸 with 牛黃 added and the weights altered — the leaf says so itself, pointing back to 鎭驚丸 in the acute fright wind section and noting that only the weights differ. The shelf is the standard one for a convulsing child: bile-processed arisaema for phlegm, bamboo silica, amber and pearl to settle, realgar and cinnabar as the minerals, musk and ox gallstone as the expensive openers, gold leaf outside. It is worth noticing what this leaf does that almost nothing else in the chapter does: it gives a dose by age. One pill at three, two at five, three to five at ten. That is a real attempt at paediatric dosing and it deserves credit for the instinct. It is still not a dose by weight, the pills are not standardised, and the thing being scaled is a mixture of mercury and arsenic. **This formula contains 朱砂 (written 辰砂), cinnabar, which is mercuric sulfide, and 雄黃, realgar, which is arsenic sulfide — 2 don 5 fun of each, the two largest doses in the formula after the arisaema. The patient is a child and the text gives no dose by weight anywhere, only a pill count by age. Mercury injures a developing nervous system irreversibly at doses that leave an adult unharmed, and arsenic is a cumulative poison and a carcinogen. 天南星 is toxic raw. 麝香 is musk, taken from a CITES Appendix I deer; it has no useful pharmacological action and the demand this tradition created is why the musk deer are endangered. The gold leaf is inert and harmless. This must not be prepared.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
