Thousand Gold Powder for Fright Wind

Cheongeum-san

한글 천금산 (소아)  ·  Pinyin Qian Jin San

千金散

The bare id `cheongeum-san` is **not** free: it belongs to 淸音散 of 醫鑑, whose romanisation is identical. That collision is why the 諸瘡門 千金散 carries a chapter tail as `cheongeum-san-jechang`, and this leaf needs one for the same reason as well as for the homonym. The 諸瘡門 formula is a quite different prescription — 乳香ㆍ沒藥ㆍ血竭ㆍ雄黃ㆍ杏仁ㆍ輕粉ㆍ孩兒茶ㆍ白礬ㆍ膽礬ㆍ麝香, an external dusting powder for syphilitic sores — and shares no ingredient with this one. The transcription writes 為 for 爲; the corpus form is printed.

Clinical commentary 臨床

Ten drugs weighed in fun and ri, and the whole dose is five to seven ri — a fifth of a gram of powder stirred into a mint and rush-pith decoction. It is among the smallest doses in the chapter, which matters, because three of the ten items are a mercury compound, a scorpion and bile-processed arisaema. The claim attached to it is the strongest in the section: 雖至死, 但灌藥下咽卽活 — even at the point of death, the moment it passes the throat the child revives. Read against what the two conditions actually are, the claim is self-explaining. A febrile seizure in a previously well child stops by itself within a few minutes, so anything given while it is happening appears to have stopped it. A child dying of dehydration after a long diarrhoea does not revive on a fifth of a gram of powder; the statement of what that child needed is given with 雙金丸. 痰喘, phlegm with laboured breathing, is the part worth taking seriously as a sign — in a child it points at bronchiolitis, pneumonia or an obstructed airway, and that is an oxygen and antibiotic problem. **This formula contains 朱砂, cinnabar, which is mercuric sulfide, at 4 fun — one of the three largest items. It contains 全蝎, whole scorpion, whose venom is a neurotoxin and whose dose cannot be measured, and 天南星, which is toxic raw. The patient is a child and the text gives no dose by weight anywhere. Mercury injures a developing nervous system irreversibly at doses that leave an adult unharmed. This must not be prepared.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
