Double Resolution Powder

Ssanghae-san

한글 쌍해산 (옹저)  ·  Pinyin Shuang Jie San

雙解散

The Bogam carries two formulas called 雙解散; the Summer Heat chapter's 河間 formula, registered as ssanghae-san, is Igwon-san and Bangpung-tongseong-san compounded, and this 得效 leaf from the 便癰 section shares with it only rhubarb, white peony and licorice, so this one takes the chapter tail. 頭末 is the first, finest powder sifted from the fried morning glory seed. The transcription runs 水煎服空心 together; it is punctuated here as 水煎服, 空心.

Translation

Treats groin toxin. Rhubarb and black morning glory seed, fried and taken as the first powder, 1 don 5 fun each; cinnamon bark, white peony, alisma and peach kernel, 1 don each; licorice, 7 fun. Cut the above, make one packet, add five slices of ginger, decoct in water and take on an empty stomach. First the urine runs freely, which is the heat leaving by the urine; afterwards the stool passes, and all of that is the thick toxin. — Deukhyo 得效 (Donguibogam · Ongjeo)

Clinical commentary 臨床

The name here means something different from the Hagan formula's double resolution of exterior and interior: in this leaf the two ways opened are the urinary and the faecal, and the leaf says so in its own closing sentence — the urine runs first and that is the heat leaving, then the stool passes and that is the thick toxin. Rhubarb and black morning glory seed drive the bowel, alisma takes the water out, peach kernel breaks the blood, white peony and licorice moderate, and a small amount of cinnamon bark warms and opens the way so that the cold draining drugs can reach a region the section calls rich in blood and remote. The chapter's first line of treatment for 便癰 is Oryeong-san with rhubarb added, or this; which is to say the first move against a groin abscess was to drain it inward through urine and stool before any thought of cutting. **Black morning glory seed is a drastic purgative and rhubarb a strong one; neither is for a depleted patient, and both are contraindicated in pregnancy. An inguinal abscess that has pointed needs drainage and antibiotics, and a bubo following a genital ulcer needs testing for sexually transmitted infection.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
