Translation

Treats sudden wind stroke with loss of consciousness; deviation of the mouth and eye, paralysis, loss of speech, numbness and dizziness; the early stroke with no sweating and a replete exterior; and treats every wind pattern. Saposhnikovia, 1.5 don; stephania, cassia bark, apricot kernel, scutellaria, white peony, ginseng, chuanxiong, ephedra and licorice, 1 don each; blast-fried aconite, 5 fun. Cut up, make one packet, add three slices of ginger and two jujubes, and decoct in water. One version has no stephania or aconite and has tangkuei and gypsum; where there is heat, use typhonium. Whenever in wind stroke the six pulses are floating and tight, the wind qi excessively exuberant, the heart fire suddenly risen and phlegm drool congested and dammed within the channels and network vessels, Soseungmyeong-tang should be used. Aconite is used because, endowed with a bold and vigorous substance and having the force of one who storms a pass and seizes command, it can lead ginseng and its kind to travel together through the twelve channels, so as to pursue and restore the original yang that has been scattered and lost; and it leads ephedra, saposhnikovia, apricot kernel and their kind to break out at the exterior and open the interstices, so as to drive off the wind cold that is in the exterior; and it leads tangkuei and chuanxiong into the blood aspect to move and nourish the blood, so as to enrich the true yin that has been depleted. Gypsum and anemarrhena may be added to bring down stomach fire, or scutellaria to clear lung metal. If the disease's force has somewhat retreated and the spirit somewhat recovered, one should at once change to Danggye's method, using prescriptions that supplement qi and blood and clear phlegm, so as to regulate and nourish the root qi. This is treating the branch when the case is urgent, and then treating the root by way of the branch. — Uihak Immun 入門 · Uihak Jeongjeon 正傳 (Donguibogam · Pung)

Clinical commentary 臨床

The most important formula in the chapter and the one the whole wind-stroke literature is organized around. Its construction is a deliberate compromise: ephedra, saposhnikovia and apricot kernel to break out at the surface; aconite and cassia to hold the yang; ginseng and licorice to support; tangkuei and chuanxiong for the blood; and scutellaria alone to keep the whole from running hot. The Jeongjeon's commentary explains every one of those choices and then does something rarer — it sets a stopping point. Once the patient has come round, change to Danggye's supplementing method; this formula treats the branch, and the branch only. Sweating and warming an acute stroke patient is of course the wrong treatment by modern lights; what is worth keeping is the discipline of a formula that names its own term of use. — Eui Young (Justin) Chung, L.Ac., Dipl.
