加減續命湯

Not an independent composition but the six-channel modification table for 小續命湯, naming six variants (麻黃續命, 桂枝續命, 白虎續命, 附子續命 and 羌活連翹續命); the Bogam prints it as a formula entry, so it is registered here under its own name.

Translation

Treats wind striking the bowels. People today do not distinguish exterior from interior or vacuity from repletion, and so Zhang Yilao handed down to Dongwon the method of modification by the six channels. For greater yang wind stroke with no sweating and aversion to cold, Ephedra Prolong Life governs it: double the ephedra, saposhnikovia and apricot kernel of the base formula. With sweating and aversion to wind, Cassia Twig Prolong Life governs it: double the cassia twig, peony and apricot kernel. For yang brightness wind stroke with no sweating, a hot body and no aversion to cold, White Tiger Prolong Life governs it: double the cassia twig and scutellaria and add 1 don 4 fun of pueraria. For greater yin wind stroke with no sweating and a cold body, Aconite Prolong Life governs it: double the aconite, add 2 don 1 fun of licorice and 7 fun of dried ginger. For lesser yin wind stroke with sweating and no fever, Cassia Twig Prolong Life governs it: double the cassia twig, aconite and licorice. Where the six channels are confounded, attach it to the lesser yang and reverting yin: whether there is cramping pain of the limbs and joints or numbness and insensibility, Notopterygium and Forsythia Prolong Life governs it — to 1 nyang of the base formula add 1 don of notopterygium and 1.5 don of forsythia. — Uihak Jeongjeon 正傳 (Donguibogam · Pung)

Clinical commentary 臨床

A table rather than a formula, and the opening sentence says why it exists: people today do not distinguish exterior from interior or vacuity from repletion. Zhang Yilao's answer was to take one base prescription and bind it to the six-channel system, so that the same drugs serve six presentations according to which pair of signs the patient shows — sweating or not, fever or not, cold body or hot. Whatever one makes of the channels, the clinical variables it is sorting on are the right ones, and the discipline of changing the ratio rather than the prescription is the chapter's best habit. — Eui Young (Justin) Chung, L.Ac., Dipl.
