Clinical commentary 臨床

The frame is the four-substance group with the three yellows laid over it: tangkuei tails, chuanxiong, red peony and fresh rehmannia cooling and moving the blood, scutellaria, coptis and phellodendron taking out the heat, and saposhnikovia, notopterygium, cimicifuga, pueraria and cicada shell pushing the eruption outward. Honeysuckle and forsythia are the pair the chapter reaches for in any toxic sore. The rhubarb-and-mirabilite addition is the interesting part, and the gloss is explicit that it is for the first dose only and is dropped as soon as the bowel has moved. The reasoning is that the toxin of a new eruption is still movable and should be sent out below before the surface is opened; keeping a strong purge going would only waste the patient. It is a well-judged instruction about dosing, applied to a disease it could not affect. 天疱瘡 and 楊梅瘡 are the same thing — syphilis — and the chapter knows it is passed in the bedchamber. **Mirabilite (芒硝) with rhubarb is a strong purge and will cause fluid and electrolyte loss if repeated; the leaf's own instruction to stop after one dose should be read as part of the prescription.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
