Disperse Wind and Transform Phlegm Decoction

Sopunghwadam-tang

한글 소풍화담탕  ·  Pinyin Xiao Feng Hua Tan Tang

消風化痰湯

The collation line 术 : 水(初刊本), standing on its own line at the page boundary before [037a] and so falling inside the lines of the 消解散 leaf two heads later, annotates this leaf: the word it marks is the 术 of 五片术煎服 here on page 036b, which the 初刊本 reads 水, 水煎服, decoct in water. The 消解散 leaf contains no 术 — its 蓬朮 is the other graph — so the note points back out of the leaf it sits in, as this chapter's notes repeatedly do. Under the 底本 rule the transcription's 术 is printed above and the first edition's reading recorded here; the standing variant-graph correction 术→朮 is deliberately not applied, because 术 here does not stand for the drug 朮 but is a block-cut corruption of 水 in the decocting instruction, and the translation reads it as 水. 欝 is given above as 鬱.

Clinical commentary 臨床

Nineteen drugs and a wind-phlegm reading of a lump. The 回春 builds it in two halves: typhonium, arisaema, pinellia, stiff silkworm and scorpion to break up phlegm that has set hard, and angelica dahurica, saposhnikovia, notopterygium and gastrodia to open the exterior and move wind. Forsythia and honeysuckle supply the cooling, akebia and platycodon the drainage, and gleditsia pod is the chapter's standing drug for driving a medicine into a mass. The logic is in the indication rather than the drugs. 因風痰鬱結 says the nodule came to rest where it did rather than grew there, so the treatment pushes outward and opens the channels instead of attacking the lump. The section's own instruction for a nodule on the arm reasons the same way — 二陳湯 with forsythia, saposhnikovia, chuanxiong and gleditsia thorn, 行太陰厥陰之積痰則自消, move the stagnant phlegm of the Taiyin and Jueyin channels and it goes of itself. **Contains typhonium, arisaema and pinellia, all toxic raw and requiring proper processing, and scorpion, which is venomous and cardiotoxic. This must not be prepared.** The nodules it describes are palpable soft-tissue or lymphatic swellings. The ones that matter are the hard, fixed, painless and enlarging ones, and no decoction distinguishes those from reactive nodes; that takes examination, imaging and where indicated a biopsy. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
