Donguibogam 東醫寶鑑

治耳項結核. 酒蒸大黃ㆍ白殭蠶ㆍ靑黛ㆍ牛膽南星各等分. 右爲末, 蜜丸彈子大, 食後噙化一丸. 《入門》

Translation

Treats nodules at the ear and on the nape. Wine-steamed rhubarb, stiff silkworm, indigo and ox-bile arisaema, equal parts of each. Powder the above, make honey pills the size of a bullet, and after food hold one pill in the mouth and let it dissolve. — Immun 入門 (Donguibogam · Jechang)

Clinical commentary 臨床

Four drugs in equal parts, bound with honey into a bullet-sized pill that is not swallowed but held in the mouth. 噙化 is the delivery, and it is chosen for the site: a nodule behind the ear or on the nape is treated by letting the drug dissolve slowly past the throat. Wine-steamed rhubarb and indigo are the cooling half, stiff silkworm and ox-bile arisaema the phlegm-and-wind half, which is this chapter's standing reading of 結核 — 火氣熱甚則鬱結堅硬如果中核, heat binding until it is as hard as the stone in a fruit. The section introducing it recommends it together with 海帶丸 for nodules scattered over the body, and the two are built the same way: a short list of drugs, equal parts, honey, held in the mouth. What 耳項結核 names is a palpable neck node. Most are reactive and settle on their own, which is what 不須潰破但熱氣散則自消 at the head of the section is describing.
