消解散

A collation line, 术 : 水(初刊本), is printed on its own line within this leaf's span at the page boundary before [037a]. It is apparatus and not part of this text, and the word it annotates is not in this leaf: the only 朮 here is the 蓬朮 of the drug list, a different graph, and the 术 the note marks stands on page 036b in the 五片术煎服 of 消風化痰湯 two heads earlier, where the first edition reads 水煎服. See the note on Sopunghwadam-tang. The transcription writes 蘓子 for 蘇子; the corpus prints 蘇 in 233 places against 7 and the standard form is given above. 蓬朮 is registered under 蓬朮 by way of the 莪朮 alias, and 燈心 under 燈心草.

Clinical commentary 臨床

Eighteen drugs built outward from 二陳湯 — arisaema and pinellia doubled at the head of the list, tangerine peel and licorice behind them — with bupleurum, peucedanum, vitex and pubescent angelica added to reach the side and back of the neck, and zedoary to break up what has hardened. Coptis, forsythia and red peony cool. The rush pith is there to drain downward and is the one ingredient that tells you the 回春 read this as heat with water held in the tissues. The three signs in the indication are a progression, not a list. Nodules in the throat, then a neck that will not turn, then stone-hard masses under both arms: that is enlargement spreading from one chain to the next, and it reads as generalised cervical and axillary lymphadenopathy. A neck that cannot be turned is also the sign of a deep neck-space infection, which is an emergency. **Contains arisaema, pinellia and typhonium, all toxic raw and requiring proper processing. This must not be prepared.** Hard, fixed nodes in more than one chain, in an adult, are the presentation of lymphoma until proved otherwise, and they need imaging and a biopsy rather than a decoction. Trismus, a fixed neck, fever or difficulty swallowing need urgent assessment the same day. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
