Inward-Supporting Ampelopsis Powder

Naetakbaengnyeom-san

한글 내탁백렴산  ·  Pinyin Nei Tuo Bai Lian San

內托白斂散

The transcription writes 白歛 for 白斂, the headword under which ampelopsis is registered in this corpus, and the standard form is printed above in both the formula name and the drug list. 片芩, the broad lamina grade of scutellaria, is registered under 黃芩 by way of the alias. The leaf runs the preparation after the drug, 片芩酒炒, and it is parenthesised here.

Clinical commentary 臨床

A 內托 prescription, which in this chapter's vocabulary means supporting the interior so that a sore that has broken down can close — but the composition is cooling and blood-moving rather than supplementing. Red peony, Korean angelica and chuanxiong move the blood; forsythia, scutellaria, trichosanthes root and seed and ampelopsis cool and drain; bupleurum and saposhnikovia carry the action to the side of the body, which is where the lump is. Frankincense and caltrop are there to break up what has set. 因酒怒氣 names the cause as drink and temper, and the chapter means this literally: the 腋下 is the Shaoyang and Jueyin territory that anger is held to heat. What the indication actually describes is clear enough — an armpit node that swelled, came to a head, discharged, and then would not stop. That pattern is an axillary abscess or, where it recurs in the same folds year after year, hidradenitis suppurativa. A discharging axillary sinus that has gone on for months does not close on a decoction. It needs examination, culture, antibiotics guided by it, and often surgical drainage or excision of the tract; recurrent disease in the axilla and groin is treated now with long antibiotic courses and, in severe cases, biologics and surgery. Frankincense and raw licorice are the only ingredients likely to be tolerated without consequence, and neither does anything for the sinus. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
