Translation

Treats the enduring malaria and the malarial mother mass. Amomum, areca seed, dichroa and tsaoko, 2 nyang each; sparganium, zedoary, green tangerine peel, aged tangerine peel, mume and pinellia, 1 nyang each. First take the dichroa and tsaoko and steep them overnight in one bowl each of wine and vinegar; then put in the other eight drugs and steep them together until evening, boil them dry over a charcoal fire, and powder them. With wine and vinegar half and half make a paste and roll pills the size of a firmiana seed; take thirty or forty pills washed down with plain hot water, twice a day. Taken until 8 nyang have been used, it pulls up the root of the disease. — Dansim 丹心 (Donguibogam · Haehak)

Clinical commentary 臨床

Ten drugs, named for ten generals, and the target is not the paroxysm but the mass it leaves behind. Sparganium and zedoary break the accumulation, areca seed and the two tangerine peels and amomum move the qi downward, pinellia clears the phlegm, mume astringes, and dichroa with tsaoko at double weight handle the malaria itself. The preparation is the whole method. Dichroa and tsaoko are steeped overnight in a bowl each of wine and vinegar, the other eight are added and steeped with them until evening, and the lot is boiled dry over charcoal before being powdered and bound with more wine and vinegar. Vinegar is the softening agent for a bound mass; the long steeping is also how the Dansim takes the violence out of the dichroa. The dose is small and daily, continued until 8 nyang have been eaten, which is the opposite of the single pre-attack dose most of this chapter uses. **Contains dichroa (常山), which is a powerful emetic and is hepatotoxic, and pinellia (半夏), which is toxic raw. This must not be prepared.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
