Donguibogam 東醫寶鑑

治白禿頭瘡. 羊糞燒存性爲末, 用雁油調塗, 一二次卽愈. 《醫鑑》

Translation

Treats white bald head sores. Burn sheep dung with its nature retained, powder it, work it up with goose fat and smear it on; one or two applications and it heals. — Gogeum Uigam 醫鑑 (Donguibogam · Jechang)

Clinical commentary 臨床

Two drugs and a claim of one or two applications. 白禿 — white bald — is the scaling, hair-losing patch on a child's scalp, that is, tinea capitis, and in its crusted form favus. The treatment is an ash and a fat: sheep dung charred 燒存性, burnt through but not reduced to white ash so that the drug is held to remain, and goose fat as the base. A greasy occlusive dressing would soften crust and let it be lifted, which is real benefit, and the alkaline ash would have some effect on the surface. That is as far as it goes. The fungus lives inside the hair shaft, which is why topical treatment does not cure tinea capitis even now. **Contains sheep dung. This must not be prepared or applied.** Faecal material on the broken skin of a child's scalp is a direct route to cellulitis and to tetanus, and the burning is not sterilisation in any reliable sense.
