Donguibogam 東醫寶鑑

治頭瘡. 酒當歸ㆍ白朮各一錢半, 酒芩ㆍ酒芍藥ㆍ川芎ㆍ陳皮各一錢, 酒天麻ㆍ蒼朮ㆍ蒼耳各七分半, 酒黃栢ㆍ酒甘草各四分, 防風三分. 右剉, 作一貼, 水煎, 日三服, 服後穩睡片時. 《入門》

Translation

Treats head sores. Wine-processed angelica root and white atractylodes 1 don 5 pun each, wine-processed scutellaria, wine-processed peony, chuanxiong and tangerine peel 1 don each, wine-processed gastrodia, atractylodes lancea and cocklebur 7 pun 5 ri each, wine-processed phellodendron and wine-processed liquorice 4 pun each, saposhnikovia 3 pun. Cut them up, make one packet, decoct in water and take it three times a day; after each dose sleep quietly a little while. — Uihak Immun 入門 (Donguibogam · Jechang)

Clinical commentary 臨床

The prescription for 癩頭瘡 that the chapter wants taken internally — 頭瘡宜服酒歸飮, head sores call for Jugui-eum — while the mineral powders go on the scalp from outside. Its distinctive feature is in the name: six of the twelve drugs are wine-processed. Wine is held to carry a drug upward and outward, and a disease at the top of the body is the standard reason for the processing. Angelica, peony, chuanxiong and the two cold bitters, scutellaria and phellodendron, are all treated this way, so the formula both nourishes and cools blood and sends that action to the head. Underneath the wine processing it is a plain enough prescription: 四物湯 missing rehmannia, with the two atractylodes and tangerine peel to dry damp, and gastrodia, cocklebur and saposhnikovia for wind. The closing instruction — 服後穩睡片時, sleep quietly a little while after the dose — is nursing advice, and the kind of detail the Bogam keeps. 癩頭瘡 is tinea capitis with secondary impetigo, and it was overwhelmingly a disease of children. Nothing in this formula reaches a dermatophyte inside the hair shaft; tinea capitis needs systemic antifungal treatment, griseofulvin or terbinafine, because topical treatment alone does not clear it.
