Donguibogam 東醫寶鑑

治上焦有熱ㆍ血壅腥悶ㆍ嗽聲連倂ㆍ氣不得透. 甘草一錢半, 薄荷ㆍ桔梗ㆍ川芎ㆍ防風ㆍ桑白皮ㆍ黃芩ㆍ前胡ㆍ紫蘇葉ㆍ柴胡ㆍ赤茯苓ㆍ枳殼各八分. 右剉, 作一貼, 入薑三片ㆍ棗二, 水煎服. 《直指》

Translation

Treats heat in the upper burner with the blood dammed up, a rank oppression in the chest, the cough sounding one volley upon another, and the breath unable to get through. Licorice, 1 don 5 fun; mint, platycodon, chuanxiong, siler, mulberry root bark, scutellaria, peucedanum, perilla leaf, bupleurum, red poria and bitter orange peel, 8 fun each. Cut up, make one packet, add three slices of ginger and two jujubes, and decoct in water. — Jikji 直指 (Donguibogam · Haesu)

Clinical commentary 臨床

嗽聲連倂 — the cough sounding one volley upon another — with the breath unable to get through describes paroxysmal coughing, and 腥悶, a rank oppression, is the smell of the sputum. Taken together with 血壅, the blood dammed up, the picture is of an infected chest with blood-streaked foul sputum, which a modern clinician would work up for bronchiectasis or an abscess. The formula itself is a surface-releasing one with mulberry root bark and scutellaria added to cool the lung, and it is the mildest of the three 血嗽 formulas. **Foul-smelling or blood-streaked sputum needs investigation and, usually, antibiotic treatment.** — Eui Young (Justin) Chung, L.Ac., Dipl.
